针灸联合耳穴压豆治疗带状疱疹的疗效及对患者疼痛症状、血清CXCL10、NF-κB水平的影响

Efficacy of Acupuncture and Moxibustion Combined with Auricular Acupoint Pressing with Beans in Treatment of Herpes Zoster and Its Influence on Pain Symptoms and Levels of Serum CXCL10 and NF-κB

目的探究针灸联合耳穴压豆对带状疱疹(herpes zoster,HZ)患者疗效及预后的影响。方法采用随机信封法将80例HZ患者分为研究组和对照组各40例,对照组经耳穴压豆+常规治疗方案,研究组在上述基础上联合针灸治疗。比较治疗1个月后两组疗效,治疗前及治疗1个月后两组疼痛症状[带状疱疹简明疼痛评估量表(zoster brief pain inventory,ZBPI)、疼痛视觉模拟评分(visual analogue scale,VAS)、匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)]、免疫因子[CXC趋化因子配体10(CXC chemokine ligand 10,CXCL10)水平;免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白G(immunoglobulin G,IgG)]、炎症因子[核因子-κB(nuclear factorκB,NF-κB)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)],记录两组患者治疗期间不良反应发生情况及带状疱疹后遗神经痛(postherpetic neuralgia,PHN)发生率。结果研究组治疗总有效率明显高于对照组,不良反应及PHN发生率明显低于对照组(均P<0.05)。治疗1个月后,两组各疼痛症状评分、CXCL10水平及各项炎症因子水平均较治疗前明显降低,且研究组水平明显低于对照组(P<0.05);IgA、IgG水平均较治疗前明显升高,且研究组水平明显高于对照组(P<0.05)。结论针灸联合耳穴压豆可有效缓解HZ患者疼痛症状,改善患者睡眠质量及免疫功能,减轻机体炎症反应,具有较好的安全性。

Objective To explore the influence of acupuncture and moxibustion combined with auricular acupoint pressing with beans on efficacy and prognosis in patients with herpes zoster(HZ).Methods Eighty patients with HZ were divided into study group(40 cases)and control group(40 cases)by the random envelope method.The control group was given auricular acupoint pressing with beans combined with conventional treatment regimen,while the study group was combined with acupuncture and moxibustion on the basis of the control group.The efficacy after 1 month of treatment and pain symptoms[zoster brief pain inventory(ZBPI),visual analogue scale(VAS),Pittsburgh sleep quality index(PSQI)],immune factors[CXC chemokine ligand 10(CXCL10),immunoglobulin A(IgA),immunoglobulin G(IgG)]and inflammatory factors[nuclear factorκB(NF-κB),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]before treatment and after 1 month of treatment were compared.The occurrence of adverse reactions and incidence rate of PHN in the two groups were recorded during treatment.Results The total effective rate of treatment in the study group was significantly higher than that in the control group while the incidence rates of adverse reactions and PHN were significantly lower than those in control group(all P<0.05).After 1 month of treatment,the pain symptoms scores,CXCL10 level and inflammatory factor levels in both groups were significantly reduced compared with those before treatment,and the scores and levels were significantly lower in the study group than those in the control group(P<0.05).The levels of IgA and IgG were significantly enhanced compared to those before treatment,and the levels were significantly higher in the study group than those in the control group(P<0.05).Conclusion Acupuncture and moxibustion combined with auricular acupoint pressing with beans can effectively relieve the pain symptoms of patients with HZ,improve the sleep quality and immune function and relieve the inflammatory response of body,and it has good safety.
