硫酸镁钠钾口服用浓溶液与复方聚乙二醇电解质散在无痛胃肠镜检查肠道准备中的效果对比

摘要 目的对比硫酸镁钠钾口服用浓溶液与复方聚乙二醇电解质散在无痛胃肠镜检查肠道准备中的效果。方法采用随机单盲前瞻性研究方法,将接受无痛胃肠镜检查的120例患者使用数字表法分为观察组和对照组,每组60例。观察组给予硫酸镁钠钾口服用浓溶液行肠道准备,对照组给予复方聚乙二醇电解质散行肠道准备。对比两组患者各段肠道波士顿肠道清洁度评分法(BBPS)评分、肠腔气泡评分、肠镜检查时间、肠道各种病变检出率、不良反应发生率、放弃检查率及对照组胃黏膜病变情况。结果观察组左半结肠、横结肠、右半结肠BBPS评分分别为(2.41±0.45)、(2.45±0.40)、(2.60±0.35)分,均高于对照组的(2.21±0.29)、(2.25±0.33)、(2.45±0.35)分(P<0.05);观察组肠腔气泡评分为(0.39±0.21)分,低于对照组的(0.54±0.28)分(P<0.05);观察组肠镜检查时间为(15.46±0.82)min,短于对照组的(17.61±0.73)min,差异显著(P<0.05)。两组患者息肉、恶性肿瘤、早期癌症、炎症检出率对比,无显著差异(P>0.05)。观察组复检意愿率为70.00%,高于对照组的25.00%,差异显著(P<0.05)。观察组腹痛、呕吐、腹胀、胃黏膜损伤的发生率分别为11.67%、5.00%、15.00%、3.33%,低于对照组的31.67%、20.00%、36.67%、18.33%,差异显著(P<0.05)。两组恶心发生率对比无显著差异(P>0.05)。观察组放弃检查率1.67%低于对照组的11.67%,差异显著(P<0.05)。结论使用硫酸镁钠钾口服用浓溶液作为无痛胃肠镜检查中肠道准备药物,不良反应少、复检意愿高,各段肠道清洁度优,放弃检查较少,对发现早期肠道肿瘤有一定临床作用,值得临床推广。 Objective To compare the effect of magnesium sulfate,sodium sulfate and potassium sulfate concentrate oral solution and compound polyethylene glycol electrolyte powder on bowel preparation in painless gastroenteroscopy.Methods According to randomized single-blind prospective study method,120 patients undergoing painless gastroenteroscopy were divided into an observation group and a control group according to random numerical table,with 60 cases in each group.The observation group was given magnesium sulfate,sodium sulfate and potassium sulfate concentrate oral solution as bowel preparation,and the control group was given compound polyethylene glycol electrolyte powder as bowel preparation.Both groups were compared in terms of Boston Bowel Preparation Scale(BBPS)scores of each segment,intestinal bubble score,colonoscopy time,detection rate of various intestinal lesions,incidence of adverse reactions,rate of abandonment of examination and gastric mucosal lesions.Results The BBPS scores of the left colon,transverse colon and right colon in the observation group were(2.41±0.45),(2.45±0.40)and(2.60±0.35)points,which were higher than(2.21±0.29),(2.25±0.33)and(2.45±0.35)points in the control group(P<0.05);the intestinal bubble score of(0.39±0.21)points in the observation group was lower than(0.54±0.28)points in the control group(P<0.05);the colonoscopy time of(15.46±0.82)min in the observation group was shorter than(17.61±0.73)min in the control group;the difference was significant(P<0.05).There were no significant differences in the detection rates of polyp,malignancy,early cancer and inflammation between the two groups(P>0.05).The willingness to re-examine of the observation group was 70.00%,which was higher than 25.00%of the control group,and the difference was significant(P<0.05).The incidence of abdominal pain,vomiting,abdominal distension and gastric mucosal injury in the observation group were 11.67%,5.00%,15.00%and 3.33%,which were lower than 31.67%,20.00%,36.67%,and 18.33%in the control group,and the differences were significant(P<0.05).The rate of abandonment of examination of the observation group was 1.67%,which was significantly lower than 11.67%of the control group(P<0.05).Conclusion Using magnesium sulfate,sodium sulfate and potassium sulfate concentrate oral solution as a bowel preparation drug before colonoscopy has the advantages of less adverse reactions,high willingness to re-examine,better cleanliness in each segment of the intestine,fewer abandonments of examination,and a certain clinical effect on the detection of early intestinal tumors,thus being worth promoting clinically.
