目标式护理模式对ICU心脏手术患者术后医疗器械相关性压力性损伤的影响

Effect of Targeted Nursing Model on the Prevention of Postoperative Medical Device-related Pressure Injury in ICU Patients Undergoing Cardiac Surgery

目的:探讨目标式护理模式对ICU心脏手术患者术后医疗器械相关性压力性损伤(MDRPI)的影响。方法:选取2021年1月1日~2023年2月28日接受心脏手术后入住ICU的96例患者作为研究对象,采用随机数字表法分为研究组和对照组各48例,对照组采用常规护理干预,研究组采用目标式护理模式。比较两组MDRPI风险(采用Braden压疮评估量表)、MDRPI发生率、不良反应发生率、凝血功能指标[包括凝血酶时间(TT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)]、护理满意度。结果:干预后,两组Braden评分高于干预前(P<0.05),且研究组高于对照组(P<0.05);研究组MDRPI发生率低于对照组(P<0.05);研究组不良反应发生率低于对照组(P<0.05);干预后,两组TT、PT、APTT水平均低于干预前(P<0.05),且研究组低于对照组(P<0.01);研究组护理满意度高于对照组(P<0.05)。结论:目标式护理模式在预防ICU心脏手术患者术后MDRPI效果较好,可降低MDRPI、不良反应发生率,改善凝血功能,提高护理满意度。

Objective:To explore the effect of targeted nursing model on prevention of postoperative medical device-related pressure injury(MDRPI)in ICU patients after cardiac surgery.Methods:A total of 96 patients who were admitted to the ICU after cardiac surgery from January 1,2021 to February 28,2023 were selected as the research objects.They were divided into the study group and the control group by the random number table method,with 48 cases in each group.The routine nursing care was given in the control group and the targeted nursing model was conducted in the study group.The risk of Braden pressure ulcer assessment Scale(MDRPI),the incidence of MDRPI,the incidence of adverse reactions,coagulation function including thrombin time(TT),prothrombin time(PT),activated partial thromboplastin time(APTT),and nursing satisfaction were compared between the two groups.Results:After the intervention,the Braden scores of the two groups were higher than those before the intervention(P<0.05),and the Braden score of the study group was higher than that in the control group(P<0.05).The incidence of MDRPI in the study group was lower than that in the control group(P<0.05).The incidence of adverse reactions in the study group was lower than that in the control group(P<0.05).After intervention,the levels of TT,PT and APTT in the two groups were lower than those before intervention(P<0.05),and the levels in the study group were lower than those of the control group(P<0.01).The nursing satisfaction of the study group was higher than that in the control group(P<0.05).Conclusion:The targted nursing model has a good effect on preventing MDRPI in ICU patients after cardiac surgery,which can reduce the incidence of MDRPI and the incidence of adverse reactions,and improve coagulation function and nursing satisfaction.
