良性前列腺增生患者术后尿路感染病原菌特征及危险因素分析

Analysis of Pathogenic Bacteria and Risk Factors of Postoperative Urinary Tract Infection in Patients with Benign Prostatic Hyperplasia

目的:分析良性前列腺增生(BPH)患者术后尿路感染的病原菌特征及危险因素。方法:选取2021年5月—2023年6月于阳江市人民医院泌尿外科择期行经尿道前列腺1470nm激光汽化术治疗的100例BPH患者临床资料进行分析,其中术后发生尿路感染者42例(感染组),未感染者58例(非感染组)。收集两组各项基线资料与手术资料,分析尿路感染的病原菌特征及危险因素。结果:感染组患者42例共检出病原菌57株,以革兰阴性菌多见(占61.40%),其次为革兰阳性菌(占35.09%)、真菌(占3.51%)。Logistic回归分析结果显示,吸烟时间≥10年、合并营养不良与糖尿病、术前行尿潴留导尿术及术后留置尿管时间长为术后尿路感染的独立危险因素(P<0.05)。结论:BPH患者术后尿路感染的病原菌主要为革兰阴性菌,危险因素包括吸烟、营养不良、糖尿病、术前尿潴留导尿术、术后留置尿管时间长等。

Objective:To analyze the pathogenic bacteria characteristics and risk factors of postoperative urinary tract infection in patients with benign prostatic hyperplasia(BPH).Methods:The clinical data of one hundred BPH patients who were selected to undergo one thousand four hundred and seventy nm laser vaporization of transurethral prostate in the Department of Urology of Yangjiang People's Hospital from May 2021 to June 2023 were analyzed.There were forty-two cases of urinary tract infection(infection group)and fifty-eight cases of uninfected(non-infection group).The baseline and surgical data of the two groups were collected to analyze the pathogenic bacteria characteristics and risk factors of urinary tract infection.Results:A total of 57 strains of pathogenic bacteria were detected in 42 patients in the infection group,and gram-negative bacteria were most common(61.40%),followed by gram-positive bacteria(35.09%)and fungi(3.51%).Logistic regression analysis showed that smoking age≥10 years,combined with malnutrition and diabetes,urinary retention catheterization before surgery and long catheter retention time after surgery were independent risk factors for postoperative urinary tract infection(P<0.05).Conclusion:The main pathogens of postoperative urinary tract infection in BPH patients were gram-negative bacteria,and the risk factors included smoking,malnutrition,diabetes,preoperative urinary retention catheterization,and long postoperative catheter retention.
