奥氮平联合多奈哌齐对血管性痴呆患者精神行为症状、血液流变学指标及炎症因子水平的影响

Effects of donepezil combined with olanzapine on mental and behavioral symptoms,hemorheology indexes and levels of inflammatory factors in vascular dementia patients

目的探究奥氮平联合多奈哌齐治疗伴精神行为症状血管性痴呆患者的效果。方法选取我院神经内科2020年10月~2022年10月期间98例伴精神行为症状血管性痴呆患者,根据抽签法分组。对照组49例给予奥氮平治疗,观察组49例增加盐酸多奈哌齐治疗,对比精神行为症状,血液流变学指标,炎症因子水平及不良反应。结果两组不良反应发生率无统计学意义(P>0.05);治疗后观察组简明精神病量表(BPRS),激惹性问卷(CMAI)评分,全血黏度,血细胞比容,纤维蛋白原,肿瘤坏死因子(TNF)-α,白细胞介素6(IL-6)和白细胞介素8(IL-8)水平低于对照组,痴呆简易量表(BSSD)评分高于对照组,有统计学意义(P<0.05)。结论盐酸多奈哌齐联合奥氮平对伴精神行为症状血管性痴呆患者治疗,可有效改善血流动力学,降低炎症反应和精神行为障碍,安全性高。

Objective To investigate the effect of donepezil combined with olanzapine on vascular dementia patients with psychobehavioral symptoms.Methods Ninety-eight patients with vascular dementia with psychobehavioral symptoms were selected from the Department of Neurology of our hospital from October 2020 to October 2022,and divided into groups according to the lottery method.49 cases in the control group were treated with olanzapine,while 49 cases in the observation group were treated with Donepezil hydrochloride.Mental and behavioral disorders,hemorheological indexes,inflammatory factor levels and adverse reactions were compared.Results There was no statistical significance in the incidence of adverse reactions between the two groups(P>0.05).After treatment,the levels of the brief Psychiatric Scale(BPRS),irritability questionnaire(CMAI),whole blood viscosity,hemocytosis,fibrinogen,tumor necrosis factor(TNF)-α,interleukin 6(IL-6)and interleukin 8(IL-8)in the observation group were lower than those in the control group,while the scores of the Simple Dementia Scale(BSSD)were higher than those in the control group.There was statistical significance(P<0.05).Conclusion The combination of donepezil hydrochloride and olanzapine can effectively improve hemodynamics,reduce inflammatory reactions and mental behavioral disorders in patients with vascular dementia accompanied by psychological and behavioral symptoms,and has high safety.
