复杂性区域疼痛综合征患者焦虑抑郁的现状及其影响

The status and influence of anxiety and depression in patients with complex regional pain syndrome

目的探讨复杂性区域疼痛综合征(CRPS)患者焦虑抑郁状态及其对症状及生活质量的影响。方法本研究纳入2021年2月~2023年2月60例CRPS患者以及60例年龄,性别等配对的健康体检人群,分别设为观察组与对照组,比较两组患者汉密尔顿焦虑(HAMA),抑郁量表(HAMD)评分。观察组合并焦虑或抑郁的患者设为焦虑抑郁组,其余设为非焦虑抑郁组,比较两组患者简氏McGill疼痛问卷(SF-MPQ),生活质量指数(QL-Index),分析HAMA评分,HAMD评分与SF-MPQ评分,QL-Index评分的相关性。结果观察组HAMA评分,HAMD评分均显著高于对照组(P<0.05);观察组合并焦虑或抑郁的患者共24例;焦虑抑郁组SF-MPQ评分均显著高于无焦虑抑郁组(P<0.05),焦虑抑郁组QL-Index评分显著低于无焦虑抑郁组(P<0.05);焦虑抑郁组HAMA评分,HAMD评分均与SF-MPQ评分具有正相关关系,与QL-Index评分具有负相关关系(P<0.05)。结论CRPS患者具有明显的焦虑抑郁情绪,且焦虑抑郁情绪会加重患者症状,降低生活质量。

Objective To explore the anxiety and depression status of patients with complex regional pain syndrome(CRPS)and its impact on symptoms and quality of life.Methods Sixty CRPS patients from February 2021 to February 2023,as well as a matched health examination population of 60 years old and gender were selected as the study subjects,and they were set as the observation group and the control group respectively.The Hamilton Anxiety and Depression Self Rating Scale(HAMA)and HAMD between two groups of patients were compared.Patients with anxiety or depression in the observation group were set as the anxiety and depression group,while the rest were set as the non anxiety and depression group.The Jane's McGill Pain Questionnaire(SF-MPQ)and Quality of Life Index(QL Index)between two groups of patients,and analyze the correlation between HAMA score,HAMD score,SF-MPQ score,and QL Index score.Results The HAMA score and HAMD score of the observation group were significantly higher than those of the control group(P<0.05).A total of 24 patients with anxiety or depression were observed in the combination;The SF-MPQ scores in the anxiety and depression group were significantly higher than those in the non anxiety and depression group(P<0.05).The QL Index score in the anxiety and depression group was significantly lower than that in the non anxiety and depression group(P<0.05).The HAMA score and HAMD score of the anxiety and depression group were positively correlated with the SF-MPQ score,and negatively correlated with the QL-Index score(P<0.05).Conclusion CRPS patients have significant anxiety and depression,which can exacerbate their symptoms and reduce their quality of life.
