摘要
目的分析血清APRIL、SOCS5、S100A9水平与过敏性紫癜患儿西咪替丁疗效的关系。方法将2021年5月至2023年7月我院收治的过敏性紫癜患儿106例作为观察组,同期选择健康患儿100例,作为对照组。检测患儿血清APRIL、SOCS5、S100A9水平。观察组在入院后使用西咪替丁治疗,分析治疗前后血清APRIL、SOCS5、S100A9水平变化,并根据治疗效果分为有效组(n=67)和无效组(n=39)。比较两组患儿血清APRIL、SOCS5、S100A9水平,分析三者与过敏性紫癜患儿西咪替丁疗效的相关性,利用ROC曲线分析三者与过敏性紫癜患儿西咪替丁治疗效果的预测价值。结果与对照组比,观察组治疗前APRIL、SOCS5、S100A9水平较高(P<0.05);观察组患儿治疗后,APRIL、SOCS5、S100A9水平均降低(均P<0.05)。对比有效组和无效组一般资料发现,关节痛、皮疹反复发作、腹痛、HDL、PLT、FIB具有统计学意义(P<0.05)。与有效组相比,无效组血清APRIL、SOCS5、S100A9水平更高(P<0.05)。利用Pearson分析得出,血清APRIL、SOCS5、S100A9水平与过敏性紫癜患儿西咪替丁疗效呈正相关(P<0.05)。Logistic回归分析显示,关节痛、腹痛、皮疹反复发作、FIB、HDL、PLT、APRIL、SOCS5、S100A9是影响过敏性紫癜患儿西咪替丁疗效的危险因素(OR>1)。通过ROC曲线分析显示,血清APRIL、SOCS5、S100A9联合诊断的敏感度高于单项诊断(P<0.05)。结论APRIL、SOCS5、S100A9水平在过敏性紫癜患儿体内呈高表达趋势,其变化与患儿使用西咪替丁治疗的效果相关。APRIL、SOCS5、S100A9水平可以预测患儿的治疗效果,改善患儿预后。
Objective To analyze the relationship between serum APRIL,SOCS5,S100A9 levels and the efficacy of cimetidine in children with anaphylactoid purpura.Methods A total of 106 children with anaphylactoid purpura admitted to our hospital from May 2021 to July 2023 were selected as the study group,and 100 healthy children were selected as the control group during the same period.The serum levels of APRIL,SOCS5 and S100A9 were detected.The study group was treated with cimetidine after admission.The changes of serum APRIL,SOCS5 and S100A9 levels before and after treatment were analyzed,and the study group was divided into effective group(n=67)and ineffective group(n=39)according to the treatment effect.The serum levels of APRIL,SOCS5 and S100A9 in the two groups were compared to analyze the correlation between the three and the therapeutic effect of cimetidine in children with anaphylactoid purpura.The predictive value of the three and the therapeutic effect of cimetidine in children with anaphylactoid purpura was analyzed by ROC curve.Results Compared with the control group,the levels of APRIL,SOCS5 and S100A9 were higher in the study group before treatment.After treatment,the levels of APRIL,SOCS5 and S100A9 in the study group were decreased,with statistical significance(P<0.05).Compared with the general data of the effective group and the ineffective group,it was found that the joint pain,repeated rash,abdominal pain,HDL,PLT and FIB had statistical significance(P<0.05).Compared with the effective group,the levels of serum APRIL,SOCS5 and S100A9 in the ineffective group were higher(P<0.05).According to Pearson analysis,serum APRIL,SOCS5 and S100A9 levels were positively correlated with the efficacy of cimetidine in children with anaphylactoid purpura(P<0.05).Logistic regression analysis showed that joint pain,abdominal pain,recurrent rash,FIB,HDL,PLT,APRIL,SOCS5,S100A9 were risk factors(OR>1)affecting the efficacy of cimetidine in children with purpura.ROC curve analysis showed that the sensitivity of combined diagnosis of serum APRIL,SOCS5 and S100A9 was higher than that of single diagnosis(P<0.05).Conclusions The levels of APRIL,SOCS5 and S100A9 showed a trend of high expression in children with henoch-Schonlein purpura,and the changes were related to the effect of cimetidine treatment.The levels of APRIL,SOCS5 and S100A9 can predict the therapeutic effect and improve the prognosis of children.
