血清总IgE、嗜酸性粒细胞、凝血功能指标水平对过敏性紫癜患儿并发过敏性紫癜肾炎的预测价值

Predictive value of serum total IgE,eosinophils and coagulation function indexes levels in children with Henoch-Schonlein purpura complicated with Henoch-Schonlein purpura nephritis

目的:分析血清总免疫球蛋白E(IgE)、嗜酸性粒细胞、凝血功能指标水平对过敏性紫癜(HSP)患儿并发过敏性紫癜肾炎(HSPN)的预测价值。方法:回顾性分析2020年1月至2023年1月该院收治的96例HSP患儿的临床资料,根据是否并发HSPN分为HSP组(n=65)和HSPN组(n=31),比较两组血清总IgE、嗜酸性粒细胞和凝血功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)]水平,并使用受试者工作曲线(ROC)分析其对HSP患儿并发HSPN的预测价值。结果:HSPN组血清总IgE、嗜酸性粒细胞水平均高于HSP组,PT、APTT均短于HSP组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,血清总IgE、嗜酸性粒细胞水平和PT、APTT预测HSP患儿并发HSPN的曲线下面积分别为0.795、0.710、0.774、0.752,均具有一定预测价值。结论:血清总IgE、嗜酸性粒细胞、凝血功能指标水平对HSP患儿并发HSPN均具有一定的预测价值。

Objective:To analyze predictive value of serum total immunoglobulin E(IgE),eosinophils and coagulation function indexes levels in children with Henoch-Schonlein purpura(HSP)complicated with Henoch-Schonlein purpura nephritis(HSPN).Methods:The clinical data of 96 children with HSP admitted to this hospital from January 2020 to January 2023 were retrospectively analyzed.According to the presence or absence of HSPN,they were divided into HSP group(n=65)and HSPN group(n=31).The levels of serum total IgE,eosinophils and coagulation function indexes[prothrombin time(PT),activated partial thromboplastin time(APTT)]were compared between the two groups.The receiver operating curve(ROC)was used to analyze their predictive value for HSPN in the children with HSP.Results:The levels of serum total IgE and eosinophils in HSPN group were higher than those in HSP group,the PT and APTT were shorter than those in HSP group,and the differences were statistically significant(P<0.05).The ROC curve analysis showed that the areas under the curve of the levels of serum total IgE,eosinophil level,PT and APTT in the prediction of HSPN in the children with HSP were 0.795,0.710,0.774 and 0.752,separately,all of which had certain predictive value.Conclusions:The serum total IgE,eosinophils and coagulation function indexes levels has certain predictive value for HSPN in the children with HSP.
