从瘀、郁、虚理论探讨勃起功能障碍的病机特点及临床实践

Analysis of pathogenic characteristics and clinical practice of erectile dysfunction based on the theory of stasis,depression,and deficiency

勃起功能障碍(ED)的病因复杂,证候以复合证型为多见,这在一定程度上影响了中医的临床诊疗。本文基于中医学“瘀、郁、虚”理论及现代医学相关研究,认为血瘀为ED发病的核心病机,肝郁为ED发病的重要因素,肾虚为ED发病的内在趋势。活血通络起痿汤是团队针对ED“血瘀、肝郁、肾虚”这一基本病理变化而凝练的协定方,临床疗效显著。同时,结合团队长期的临床实践,提出ED的三大治疗策略,即治病与治人相结合、治本与治标相结合、中药与西药相结合,多法合用,可有效提高ED的整体疗效,值得推广运用。

The etiology of erectile dysfunction(ED)is complex,and complex syndromes are common,which may affect the clinical diagnosis and treatment of traditional Chinese medicine to some extent.Based on the theory of“stasis,depression,and deficiency”in traditional Chinese medicine,as well as modern medical research,this article proposes that blood stasis is the core pathogenesis of ED,liver depression is an important factor for ED occurrence,and kidney deficiency is the inherent trend of ED ocurrence.Huoxue Tongluo Qiwei Tang is an agreement formulated by the team to address the basic pathological changes of ED,including blood stasis,liver depression,and kidney deficiency.Its clinical efficacy is significant.Meanwhile,based on our team's long-term clinical practice,we develop and propose three treatment strategies for ED,namely the combination of treating the disease and treating the patient,the combination of treating the root cause and treating the symptoms,the combination of traditional Chinese medicine and Western medicine,and the combination of multiple methods,which can effectively improve the overall therapeutic effect of ED and are worthy of promotion and application.
