针状肾镜辅助标准通道经皮肾镜碎石取石术治疗重度Ⅷ因子缺乏孤立肾完全鹿角型结石一例并文献复习

Needle-perc-assisted standard percutaneous nephrolithotomy for the treatment of patients with complete staghorn stones in a solitary kidney combined with severe factorⅧdeficiency(hemophilia type A):a case report and literature review

经皮肾镜碎石取石术(percutaneous nephrolithotomy,PCNL是治疗鹿角型结石的首选方法,但对于合并凝血功能障碍的患者,出血风险明显增加,严重者危及生命。本文报道了全超声引导下针状肾镜(Needleperc)辅助标准通道经皮肾镜碎石取石术治疗合并重度Ⅷ因子缺乏A型血友病的孤立肾完全鹿角型结石的患者1例,通过查阅相关文献,探讨经皮肾镜手术对合并凝血功能障碍患者的安全性以及针状肾镜的临床应用价值。

Objective:Percutaneous nephrolithotomy(PCNL)is the preferred method for the treatment of staghorn stones,but for patients with coagulation dysfunction,the risk of bleeding is significantly increased,and severe cases may be life-threatening.A case of solitary kidney complete staghorn stones complicated with severe factorⅧdeficiency hemophilia A and total ultrasound-guided Needle-perc-assisted standard PCNL was reported.To investigate the safety of PCNL in patients combined with coagulation dysfunction and the clinical value of needle-perc were analyzed by reviewing relevant literature.
