磁共振成像在前列腺癌图像引导放疗中的应用进展

Application progress of magnetic resonance imaging in image guided radiotherapy for prostate cancer

随着前列腺癌(PCa)图像引导放疗(IGRT)软、硬件水平的提高,临床价值可观的辅助技术层出不穷。这些辅助技术中基于黄金基准标志物(GFM)的图像匹配和校准提高了PCa IGRT中靶区位置准确度,使患者得到了更高的治疗获益。通过注入水凝胶增加了前列腺同直肠前壁距离,从而降低了直肠毒性。MRI上GFM辨识准确度较高,可满足CT-MRI影像融合和单独基于MRI的PCa IGRT要求。MRI对水凝胶注入评价较全面,可保证水凝胶对直肠的保护效果。MRI的应用对提高放疗疗效和患者生活质量具有重要意义,已成为PCa IGRT中的研究热点。

Numerous auxiliary technologies with significant clinical value emerged with the improvement of software and hardware level in image guided radiotherapy(IGRT)for prostate cancer(PCa).The image matching and calibration based on the gold fiducial marker(GFM)in the these ancillary technology had improved the accuracy of target location in PCa IGRT.It enabled patients to achieve higher treatment gains.The distance between the prostate and the anterior wall of the rectum was increased by hydrogel injection,reducing rectal toxicity.The identification accuracy of GFM on MRI was high,which could meet the requirement of CT-MRI image fusion and PCa IGRT based only on MRI.The evaluation of hydrogel injection on MRI was relatively comprehensive,which could ensure the protective effect of hydrogel on rectum.The application of MRI was of great significance in improving radiotherapy efficacy and patient quality of life.Making it a focus of research in PCa IGRT.
