不孕不育夫妇中男性患者生殖道支原体感染及其药敏结果分析

Analysis of Mycoplasma infection in genital tract and drug sensitivity in male patients in infertility couples

目的 调查不孕不育夫妇中男性患者生殖道支原体感染状况并分析其药物敏感性,为临床治疗提供依据。方法 选取2022年1月至2023年12月就诊于中山大学附属第六医院生殖医学中心的不孕不育夫妇中15 205例男性患者,分析生殖道支原体感染情况及药敏结果。结果 15 205例患者中,共检出1437例生殖道支原体阳性,感染率为9.45%,其中解脲支原体(Uu)感染最为常见,占83.51%。26~35岁生殖道支原体感染率最高(62.63%)。Uu对原始霉素的敏感率最高,其次为交沙霉素和强力霉素;对环丙沙星的耐药率最高,其次为氧氟沙星。人型支原体(Mh)对强力霉素、原始霉素和交沙霉素的敏感率最高;对红霉素的耐药率最高,其次为氧氟沙星。Uu+Mh对原始霉素的敏感率最高,其次为交沙霉素;对环丙沙星的耐药率最高,其次为氧氟沙星。结论 不孕不育夫妇中男性患者Uu感染率较高,且多集中在26~35岁,生殖道支原体对交沙霉素、强力霉素及原始霉素敏感,可作为首选治疗药物。

Objective To investigate the Mycoplasma infection in genital tract state of male patients with infertility couples and analyze drug sensitivity,so as to provide evidence for clinical treatment.Methods A total of 15205 male patients in infertility couples who were admitted to Center for Reproductive Medicine,the Sixth Affiliated Hospital,Sun Yat-sen University from January 2022 to December 2023 were selected to analyze Mycoplasma infection in genital tract state and drug sensitivity results.Results Among the 15205 patients,1437 were positive for Mycoplasma in the genital tract,with an infection rate of 9.45%,and Ureaplasma urealyticum(Uu)infection was the most common,accounting for 83.51%.The infection rate of Mycoplasma in the genital tract was the highest in 26-35 years old(62.63%).The sensitive rate of Uu to Pristinamycin was the highest,followed by Josamycin and Doxycycline.The drug resistance rate to Ciprofloxacin was the highest,followed by Ofloxacin.The sensitive rate of Mycoplasma hominis(Mh)to Doxycycline,Pristinamycin,and Josamycin was the highest.The drug resistance rate to Erythromycin was the highest,followed by Ofloxacin.The sensitive rate of Uu+Mh to Pristinamycin was the highest,followed by Josamycin.The drug resistance rate to Ciprofloxacin was the highest,followed by Ofloxacin.Conclusion The Uu infection rate of male patients in infertility couples is high,and most of them are 26-35 years old.Genital Mycoplasma is sensitive to Josamycin,Doxycycline,and Pristinamycin,which can be used as the first choice drug of treatment.
