老年维持性腹膜透析病人Triangle分层分级管理体系的构建

Construction of Triangle hierarchical classification management system for elderly patients with maintenance peritoneal dialysis

目的:构建适合老年维持性腹膜透析病人的分层分级管理体系,为老年腹膜透析病人的管理提供参考依据。方法:以Triangle分层分级管理模式为理论依据进行文献检索,通过半结构式访谈对15名从事腹膜透析及肾脏病临床管理的医护专家进行咨询,构建老年腹膜透析病人分层分级管理内容框架。结果:2轮问卷回收率均达到100%,2轮专家权威系数分别为0.870和0.923,肯德尔协调系数分别为0.318和0.433,第2轮专家函询中各指标重要性赋分均数为4.07~4.93,变异系数为0.05~0.15。最终构建包括6个1级指标,45个2级指标的老年维持性腹膜透析病人Triangle分层分级管理体系。结论:该研究构建的老年维持性腹膜透析病人Triangle分层分级管理体系内容具有科学性和可靠性,可为老年维持性腹膜透析病人的管理提供依据,具有理论和实际意义。

Objective:To construct a hierarchical classification management system suitable for elderly patients with maintenance peritoneal dialysis,and provide a reference for the management of elderly patients with peritoneal dialysis.Methods:Based on the Triangle hierarchical classification management model,literature search was conducted,and 15 medical experts engaged in the clinical management of peritoneal dialysis and kidney disease were consulted through semi-structured interviews,in order to construct a hierarchical management content framework for elderly peritoneal dialysis patients.Results:The recovery rates of the two rounds of questionnaires reached 100%,the expert authority coefficients of the two rounds were 0.870 and 0.923,and Kendall′s W were 0.318 and 0.433,respectively.In the second round of expert correspondence,the mean scores of importance of each indicator ranged from 4.07 to 4.93,and the coefficient of variation ranged from 0.05 to 0.15.Finally,the Triangle hierarchical classification management system for elderly maintenance peritoneal dialysis patients was constructed,including 6 first-level indicators and 45 second-level indicators.Conclusions:The contents of the Triangle hierarchical classification management system for elderly maintenance peritoneal dialysis patients constructed in this study were scientific and reliable,and could provide a basis for the management of elderly maintenance peritoneal dialysis patients,which had theoretical and practical significance.
