儿童支原体大叶性肺炎三种病原体检测方法的对比分析

摘要 目的研究肺炎支原体感染所致大叶性肺炎的早期实验室检查结果,探讨相关指标的疾病预测价值。方法回顾性分析南京医科大学第二附属医院2023年1月-8月收治的疑似肺炎支原体感染致大叶性肺炎患儿的实验室检测结果,分析三种支原体病原体和血清学检测方法对儿童支原体大叶性肺炎的早期诊断意义。结果核酸检测法的MP阳性检出率最高,化学发光法、间接免疫荧光法和核酸检测法的MP检测阳性率分别为37.78%、28.89%和82.22%;三种检测方法的一致性较差,两两比较的Kappa值分别为0.089、-0.444和-0.533。不同性别患儿化学发光法MP检测结果比较,差异有统计学意义(P<0.05);不同年龄、不同拷贝数三种检测方法MP阳性检出率比较,差异无统计学意义(P>0.05)。结论同步进行核酸和抗体检测有助于及早、准确诊断儿童支原体肺炎,为该疾病的早期诊断和治疗提供依据。 Objective To study the early laboratory examination results of lobar pneumonia caused by Mycoplasma pneumoniae infection,and to explore the predictive value of related indicators.Methods The laboratory test results of children with lobar pneumonia caused by suspected mycoplasma pneumoniae infection admitted to the Second Affiliated Hospital of Nanjing Medical University from January to August 2023 were retrospectively analyzed.The significance of three mycoplasma pathogens and serological detection methods in the early diagnosis of mycoplasma lobar pneumonia in children was analyzed.Results The positive detection rate of MP by nucleic acid detection method was the highest,the positive detection rates of MP by chemiluminescence method,indirect immunofluorescence method and nucleic acid detection method were 37.78%,28.89%and 82.22%,respectively,the consistency of the three detection methods was poor,and the Kappa values of pairwise comparison were 0.089,-0.444 and-0.533,respectively.There was a statistically significant difference in the results of MP detection by chemiluminescence in children of different genders(P<0.05).Conclusion Simultaneous nucleic acid and antibody detection is helpful for early and accurate diagnosis of mycoplasma pneumonia in children,and provides a basis for early diagnosis and treatment of the disease.
