芍药甘草汤加味联合硝苯地平缓释片治疗尿毒症透析患者头痛的效果

Effects of modified Shaoyao Gancao Decoction combined with extended release nifedipine tablets in the treatment of headache in uremic dialysis patients

目的探讨芍药甘草汤加味联合硝苯地平缓释片治疗尿毒症透析患者头痛的效果。方法选择2022年5月至2024年2月山西省平陆县人民医院就诊的76例尿毒症透析合并头痛患者,按治疗方法分为2组,2组均规律行血液透析,对照组(38例)予硝苯地平缓释片治疗,观察组(38例)予芍药甘草汤加味联合硝苯地平缓释片治疗。治疗前后测定血压,记录患者近1个月内的头痛程度和发作次数,评价临床疗效,同时评估残余肾功能,评价安全性。结果治疗后观察组收缩压(SBP)为(139±4)mmHg,舒张压(DBP)为(78±5)mmHg,对照组为(143±6)mmHg,(82±6)mmHg,比较差异具有统计学意义(t=2.983、3.000,P<0.05)。治疗后观察组头痛程度为(1.7±0.5)分,发作次数为(2.5±0.7)次,对照组分别为(2.2±0.9)分,(4.9±1.6)次,比较差异具有统计学意义(t=2.508、8.141,P<0.05)。观察组总有效率为89%,对照组为66%,比较差异具有统计学意义(χ^(2)=6.138,P<0.05)。治疗后观察组血肌酐(Scr)为(451±39)μmol/L,对照组为(466±37)μmol/L,比较差异具有统计学意义(t=4.089,P<0.05)。结论芍药甘草汤加味对尿毒症透析患者的血压水平具有良好的调控作用,联合硝苯地平缓释片治疗可协同增效,有效缓解头痛症状,且无明显不良反应。

Objective To investigate the effect of modified Shaoyao Gancao decoction combined with ex-tended release nifedipine tablets in the treatment of headache in uremic dialysis patients.Methods A total of 76 puremic dialysis patients with hypertension in Pinglu County People′s Hospital from May 2022 to February 2024 were divided into a control group(n=38)and an observation group(n=38).Both groups underwent regular hemodialysis.The control group was treated with extended release nifedipine tablets,while the observation group was treated with modified Shaoyao Gancao decoction combined with extended release nifedipine tablets.Measured the blood pressure of all patients before and after treatment,recorded the severity and frequency of headaches in the past month,evaluated clinical efficacy,assessed residual renal function,and evaluated the safety.Results After treatment,the systolic blood pressure(SBP)and diastolic blood pressure(DBP)in the observation group were(139±4)mmHg and(78±5)mmHg,respectively,while those in the control group were(143±6)mmHg and(82±6)mmHg,respectively.There were statistically significant differences between the two groups(t=2.983,3.000,P<0.05).After treatment,the headache severity and frequency in the observation group were(1.7±0.5)points and(2.5±0.7)times,respectively,while those in the control group were(2.2±0.9)points and(4.9±1.6)times,respec-tively.There was a statistically significant difference between the two groups(t=2.508,8.141,P<0.05).The total effective rate was 89%in the observation group and 66%in the control group,with a statistically significant dif-ference between the two groups(χ^(2)=6.138,P<0.05).After treatment,the serum creatinine(Scr)level in the obser-vation group was(451±39)μmol/L,while that in the control group was(466±37)μmol/L,showing a statistically significant difference between the two groups(t=4.089,P<0.05).Conclusions Modified Shaoyao Gancao Decoc-tion has a good regulatory effect on blood pressure levels in uremic dialysis patients.When combined with extend-ed release nifedipine tablets,it can have a synergistic effect,effectively alleviate headache symptoms,and have no obvious adverse reactions.
