瑞格列奈联合二甲双胍治疗血糖控制不佳2型糖尿病的临床研究

Clinical study of repaglinide combined with metformin in the treatment of type 2 diabetes mellitus with poor glycemic control

目的:观察血糖控制不佳2型糖尿病(Diabetes mellitus type 2,T2DM)患者采用瑞格列奈联合二甲双胍治疗的效果。方法:选取2023年1月至2024年1月期间于本院诊疗的120例血糖控制不佳T2DM患者作为研究对象。随机将患者分为对照组和观察组,每组各60例。对照组予采用二甲双胍联合胰岛素注射治疗;观察组在对照组基础上采用瑞格列奈治疗。分析比较两组的血糖水平[空腹血糖(Fasting blood glucose,FBG)、餐后2 h血糖(2 h blood glucose,2hPBG)、糖化血红蛋白(Hemoglobin A1C,HbA1c)水平]、胰岛相关功能[胰岛素抵抗指数(Homeostatic Model Assessment for Insulin Resistance,HOMA-IR)和β细胞功能指数(Homeostasis model assessment-β,HOMA-β)]以及不良反应发生情况。结果:治疗后,观察组的FBG、2hPBG、HbA1c和HOMA-IR均明显低于对照组(P<0.05);且观察组HOMA-β高于对照组(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义。结论:瑞格列奈联合二甲双胍治疗血糖控制不佳的T2DM患者,对患者胰岛功能的影响更好,能获得更理想的血糖控制效果,且不会增加用药不良反应,安全可靠。

Objective:To observe the efficacy of repaglinide combined with metformin in patients with type 2 diabetes mellitus(T2DM)with poor glycemic control.Methods:120 T2DM patients with poor glycemic control were selected from January 2023 to January 2024 in our hospital.Patients were randomly divided into control group and observation group,with 60 cases in each group.The control group was treated with metformin combined with insulin injection.Observation group was treated with repaglinide on the basis of control group.The blood glucose level,islet related function[(insulin resistance index(HOMA-IR)andβcell function index(HOMA-β))]and the occurrence of adverse reactions were analyzed and compared between the two groups.Results:After treatment,the levels of Fasting blood glucose(FBG),2 hours blood glucose(2hPBG),Hemoglobin A1C(HbA1c),and HOMA-IR in the observation group were significantly lower than those in the control group(P<0.05);and HOMA-βof observation group was significantly higher than that of control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:Repaglinide combined with metformin in the treatment of T2DM patients with poor glycemic control can improve the islet function and improve the glycemic control effect,with high safety.
