腹腔镜卵巢囊肿剥除术中应用不同气腹压力对术中呼吸循环系统及术后并发症发生风险的影响

摘要 目的:探讨腹腔镜卵巢囊肿剥除术中应用不同气腹压力对术中呼吸循环系统及术后并发症发生风险的影响。方法:将选自2019年3月-2023年7月在本院接受腹腔镜卵巢囊肿剥除术的80例患者进行研究,采用简单随机分组将患者分为高压组(14~15mmHg)和低压组(10~11mmHg),每组各40例。比较2组术中呼吸循环系统及术后并发症发生的情况。结果:气腹后25min2组患者平均动脉压(MAP)、心率(HR)及呼气末CO_(2)分压(PETCO_(2))较气腹前均提高,且高压组高于低压组(P<0.05);术后,高压组并发症出现8例(20.00%)高于低压组2例(5.00%)(P<0.05)。结论:腹腔镜卵巢囊肿剥除术中应用低气腹压力降低对患者呼吸循环系统影响的同时可减少并发症的发生。 Objective:To investigate the effects of different pneumoperitoneum pressures used in laparoscopic ovarian cystectomy on intraoperative respiratory and circulatory system and the risk of postoperative complications.Methods:Eighty patients who underwent laparoscopic ovarian cystectomy in the hospital from March 2019 to July 2023 were included in the study.They were randomly assigned to the high-pressure group(14-15mmHg)and the low-pressure group(10-11mmHg),with 40 patients in each group.Intraoperative respiratory and circulatory system and the incidence of postoperative complications were compared between the two groups.Results:At 25 min after pneumoperitoneum,there were increases in mean arterial pressure(MAP),heart rate(HR)and end-tidal carbon dioxide pressure(PETCO_(2))in both groups.Besides,above indicators in the high-pressure group were higher than those in the low-pressure group(P<0.05).After surgery,8(20.00%)cases of complications occurred in the high-pressure group,more than 2(5.00%)cases in the low-pressure group(P<0.05).Conclusion:Using low pneumoperitoneum pressure during laparoscopic ovarian cystectomy can reduce the impact on the patient's respiratory and circulatory system,and reduce the incidence of complications.
