氧化平衡评分与哮喘患者全因死亡和心血管事件死亡的关联:NHANES数据分析

摘要 目的:探讨氧化平衡评分(OBS)与哮喘患者全因死亡及心血管事件死亡的关联。方法:使用1999至2018年美国全国健康与营养检查调查(NHANES)的数据,选择16种营养素和4种生活方式因素进行OBS评分,并根据三分位数分为3组(Q1、Q2、Q3)组。采用Cox回归分析OBS与哮喘患者全因死亡及心血管事件死亡的关系,并采用亚组分析和交互作用测试评价这种关联在不同群体中是否稳定。结果:研究共纳入6739人。Cox回归分析显示,在调整混杂因素后,以Q1组为参照,Q2组和Q3组个体全因死亡风险降低(HR=0.65,95%CI:0.49~0.86;HR=0.53,95%CI:0.37~0.77),Q3组心血管事件死亡风险降低(HR=0.45,95%CI:0.27~0.76)。性别及是否患有慢性阻塞性肺病、高血压、冠状动脉疾病、卒中、糖尿病在OBS与全因死亡的关联中无交互作用(P>0.05)。结论:高OBS是哮喘人群全因死亡和心血管事件死亡的保护因素。 Aim:To explore the association of oxidative balance score(OBS)with all-cause death and cardiovascular event death in patients with asthma.Methods:Data from the National Health and Nutrition Examination Survey(NHANES)from 1999 to 2018 were used in this study.Sixteen dietary factors and 4 lifestyle factors were selected for OBS scoring,and the enrolled asthma patients were allocated into 3 groups(Q1,Q2,Q3 groups)according to the tertiles of OBS.Cox regression analysis was employed to investigate the association between OBS and all-cause death as well as cardiovascular event death in the asthmatic population.Subgroup analysis and interaction tests were used to examine whether this association was stable in different populations.Results:This study included 6739 participants.Cox regression analysis showed that after adjusting confounding factors,individuals in Q2 and Q3 groups had lower risk of all-cause death(HR=0.65,95%CI:0.49-0.86;HR=0.53,95%CI:0.37-0.77),and those in Q3 group had lower risk of cardiovascular event death(HR=0.45,95%CI:0.27-0.76).Subgroup analysis and interaction tests indicated that gender,chronic obstructive pulmonary disease,hypertension,coronary artery disease,stroke,or diabetes had no significant interactions between OBS and all-cause death(P>0.05).Conclusion:High OBS is a protective factor against all-cause death and cardiovascular event death in the asthmatic population.
