YM-W型胃动力治疗仪结合大柴胡汤保留灌肠治疗急性胰腺炎(腑实热结型)的临床效果

Clinical Effect of YM-W Type Gastric Dynamic Therapy Instrument Combined with Dachaihu Decoction Retention Enema in the Treatment of Acute Pancreatitis(Fu-heat Junction Type)

目的 探讨YM-W型胃动力治疗仪联合大柴胡汤保留灌肠治疗急性胰腺炎(腑实热结型)的临床效果。方法 抽取2022年1月—2023年6月如东县中医院收治的61例急性胰腺炎(腑实热结型)患者为研究对象,按照治疗方法的不同分成3组。对照1组(20例)接受西医治疗,对照2组(20例)接受西医+YM-W型胃动力治疗仪治疗,观察组(21例)接受西医+YM-W型胃动力治疗仪+大柴胡汤保留灌肠治疗。对比组间患者肠功能恢复时间、改良CT严重指数(Modified CT Severity Index,MCTSI)评分、急性胰腺炎严重程度床边评分(Bedside Index for Severity in Acute Pancreatitis,BISAP)、中医证候积分和生化指标。结果 观察组腹胀、腹痛缓解时间及第1次自主排便时间分别为(3.07±0.65)d、(4.83±0.96)d、(5.25±1.07)d,短于对照2组[(6.67±1.56)d、(6.18±1.06)d、(9.33±1.26)dd]和对照1组[(8.15±1.34)d、(8.23±1.13)d、(11.35±1.20)d],且对照2组较对照1组更短,差异有统计学意义(F=89.320、54.256、141.358,P均<0.05)。治疗前,3组患者MCTSI评分、BISAP评分、中医证候积分比较,差异无统计学意义(P均>0.05);治疗后,3组上述分数均改善,且观察组改善程度较对照2组和对照1组更高,差异有统计学意义(P均<0.05)。治疗前,3组C反应蛋白、血清脂肪酶、血清淀粉酶、尿淀粉酶水平比较,差异无统计学意义(P均>0.05);治疗后,观察组上述生化指标显著降低,较对照2组和对照1组更优,且对照2组优于对照1组,差异有统计学意义(P均<0.05)。结论 给予急性胰腺炎(腑实热结型)患者YM-W型胃动力治疗仪联合大柴胡汤保留灌肠治疗效果确切,不仅可以早期缓解病情,还可促进肠功能的恢复。

Objective To investigate the clinical effect of YM-W type gastric dynamic therapy instrument combined with Dachaihu Decoction retention enema in the treatment of acute pancreatitis.Methods A total of sixty-one patients with acute pancreatitis(fu-heat junction type) admitted to Rudong County Hospital of Traditional Chinese Medicine from January 2022 to June 2023 were selected as the study objects and divided into 3groups according to different treatment methods.Control group 1(20 cases) received Western medicine treatment,control group 2(20 cases) received Western medicine +YM-W type gastric dynamic device treatment,observation group(21 cases) received Western medicine +YM-W type gastric dynamic device + Dachaihu Decoction retention enema treatment.Intestinal function recovery time,Modified CT Severity Index(MCTSI) score,Bedside Index for Severity In Acute Pancreatitis(BISAP),Traditional Chinese Medicine syndrome score and biochemical indexes were compared between groups.Results The relief time of abdominal distension,abdominal pain and first time of spontaneous defecation in observation group were(3.07±0.65) d,(4.83±0.96) d and(5.25±1.07) d,respectively,they were shorter than control group 2 [(6.67±1.56) d,(6.18±1.06) d,(9.33±1.26) d]and control 1 group [(8.15±1.34) d,(8.23±1.13) d,(11.35±1.20) d],and control group 2 was shorter than control group 1,and the difference was statistically significant(F=89.320,54.256,141.358,all P<0.05).Before treatment,there were no significant differences in MCTSI score,BISAP score and Traditional Chinese Medicine syndrome score among three groups(all P>0.05).After treatment,the above scores were improved among all groups,and the improvement degree of observation group was higher than that of control group 2 and control group 1,and the difference was statistically significant(all P<0.05).Before treatment,there were no significant differences in C-reactive protein,serum lipase,serum amylaseand urine amylase levels between three groups(all P>0.05).After treatment,the above biochemical indexes in the observation group were significantly reduced,which were better than those in control group 2 and control group 1,and control group 2 was better than control group 1,the differences were statistically significant(all P<0.05).Conclusion The effect of YM-W type gastric dynamic therapy instrument combined with Dachaihu Decoction retention enema in patients with acute pancreatitis is accurate,which can not only alleviate the condition early,but also promote the recovery of the intestinal function.
