无创呼吸机联合枸橼酸咖啡因治疗早产儿原发性呼吸暂停的临床研究

摘要 目的探讨无创呼吸机联合枸橼酸咖啡因治疗早产儿原发性呼吸暂停的临床效果。方法方便选取2021年1月—2023年9月邳州市中医院急诊收治的82例原发性呼吸暂停早产儿为研究对象,依据不同治疗方法分为对照组与研究组,每组41例,对照组采用单纯无创呼吸机辅助呼吸,研究组采用无创呼吸机辅助通气联合枸橼酸咖啡因治疗,对比两组治疗有效率、血氧指标、临床指标、并发症、24 h内呼吸暂停次数。结果治疗后,两组并发症发生率比较,差异无统计学意义(χ^(2)=0.064,P=0.800)。研究组血氧分压高于对照组,二氧化碳分压以及呼吸频率低于对照组,差异有统计学意义(P均<0.05)。研究组治疗有效率[97.56%(40/41)]高于对照组[85.37%(35/41)],差异有统计学意义(χ^(2)=3.905,P=0.048)。研究组总用氧时间、住院时间、撤机时间、呼吸暂停消失时间均短于对照组,差异有统计学意义(P均<0.05)。研究组24 h呼吸暂停发作次数少于对照组,差异有统计学意义(P<0.05)。结论针对原发性呼吸暂停早产儿采用呼吸机辅助通气联合枸橼酸咖啡治疗效果显著。 Objective To investigate the clinical effect of non-invasive ventilator combined with caffeine citrate on pri-mary apnea of premature infants.Methods A total of 82 premature infants with primary apnea admitted to the Emer-gency Department of Pizhou Hospital of Traditional Chinese Medicine from January 2021 to September 2023 were con-veniently selected as the study subjects.They were divided into control group and study group according to different treatment methods,with 41 cases in each group.The control group was treated with simple non-invasive ventilator as-sisted breathing,and the study group was treated with non-invasive ventilator assisted ventilation combined with caf-feine citrate.The treatment efficiency,blood index,clinical indicators,complications and number of apnea attack in 24 h of the two groups was compared.Results After treatment,there was no significant difference in the incidence of complications between two groups(χ^(2)=0.064,P=0.800).The blood oxygen partial pressure of the study group was higher than that of the control group,and the carbon dioxide partial pressure and respiratory rate were lower than that of the control group,the differences were statistically significant(all P<0.05).The effective rate of the study group[97.56%(40/41)]was higher than that of the control group[85.37%(35/41)],and the difference was statistically sig-nificant(χ^(2)=3.905,P=0.048).The total oxygen consumption time,hospitalization time,withdrawal time and apnea dis-appearance time of the study group were shorter than those of the control group,and the differences were statistically significant(all P<0.05).The number of apnea attacks in 24 h in the study group was less than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Ventilator assisted ventilation combined with caf-feine citrate is effective for premature infants with primary apnea.
