乙酰半胱氨酸溶液雾化吸入对COPD的治疗效果及对肺功能的影响

摘要 目的研究乙酰半胱氨酸溶液雾化吸入治疗慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者的临床效果及对肺功能的影响。方法随机选取2020年10月—2023年12月莆田平民医院内科收治的100例COPD患者为研究对象,依据不同治疗措施分为研究组和对照组,每组50例。对照组行常规对症治疗,研究组在对照组的基础上行乙酰半胱氨酸溶液雾化吸入。比较两组治疗效果、肺功能指标、T淋巴细胞亚群、炎性因子。结果治疗前,两组患者的肺功能指标、T淋巴细胞亚群、炎性指标对比,差异无统计学意义(P均>0.05);治疗后,研究组的第1秒用力呼气容积(forced expiratory volume in 1 second,FEV1)、用力肺活量(forced vital capacity,FVC)、FEV1/FVC较对照组均更高,差异有统计学意义(P均<0.05)。研究组的成熟T淋巴细胞(cluster of differentiation 3^(+),CD3^(+))、辅助性T淋巴细胞(cluster of differentiation 4^(+),CD4^(+))及CD4^(+)/CD8^(+)水平较对照组均更高,抑制/杀伤性T淋巴细胞(cluster of differentiation 8^(+),CD8^(+))水平较对照组更低,差异有统计学意义(P均<0.05)。研究组的炎性因子水平较对照组更低,差异有统计学意义(P<0.05)。治疗后,研究组的治疗总有效率96.00%(48/50)较对照组的84.00%(42/50)更高,差异有统计学意义(χ^(2)=4.000,P=0.046)。结论乙酰半胱氨酸溶液雾化吸入是一种常用的治疗COPD的方法,可积极改善患者的临床症状及肺功能,提升临床效果。 Objective To study the clinical effect of acetylcysteine solution atomization inhalation in the treatment of patients with chronic obstructive pulmonary disease(COPD)and its effect on lung function.Methods A total of 100 patients with COPD admitted to the Department of Internal Medicine of Putian Pingmin Hospital from October 2020 to December 2023 were randomly selected as the research objects.According to different treatment measures,they were divided into study group and control group,with 50 cases in each group.The control group was treated with routine symptomatic treatment,and the study group was treated with acetylcysteine solution atomization inhalation on the basis of the control group.The therapeutic effect,pulmonary function index,T lymphocyte subsets and inflammatory factors were compared between the two groups.Results Before treatment,there were no significant differences in pulmonary function indexes,T lymphocyte subsets and inflammatory indexes between the two groups(all P>0.05);after treatment,the forced expiratory volume in 1 second(FEV1),forced vital capacity(FVC)and FEV1/FVC in the study group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The levels of mature T lymphocytes(cluster of differentiation 3^(+),CD3^(+)),helper T lymphocytes(cluster of differentiation 4^(+),CD4^(+))and CD4^(+)/CD8^(+)in the study group were higher than those in the control group,and the level of inhibition/killing T lymphocytes(cluster of differentiation 8^(+),CD8^(+))was lower than that in the control group,and the differences were statistically significant(all P<0.05).The level of inflammatory factors in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the total effective rate of treatment in the study group was 96.00%(48/50)higher than 84.00%(42/50)in the control group,and the difference was statistically significant(χ^(2)=4.000,P=0.046).Conclusion Acetylcysteine solution atomization inhalation is a common method for the treatment of COPD,which can actively improve the clinical symptoms of patients with pulmonary function,improve the clinical effect.
