摘要
目的探讨参芪利水颗粒配合脐灸治疗慢性阻塞性肺疾病急性加重期(Acute exacerbation of chronic obstructivepulmonary disease,AECOPD)患者的临床疗效。方法选取2022年8月—2023年8月自河北省保定市第一中医院收治的60例AECOPD患者,按照随机数字表法分为对照组和观察组,每组各30例。对照组采用常规西医治疗方法,观察组在对照组基础上脐灸联合参芪利水颗粒治疗,治疗2周后,观察比较两组患者临床疗效、治疗安全性,治疗前后改良版英国医学研究委员会呼吸问卷(modified British medical research council,mMRC)评分、慢阻肺患者自我评估测试(COPD Assessment test,CAT)评分、血气分析指标及血清炎性因子[降钙素原(Procalcitonin,PCT)、肿瘤坏死因子-α(Tumor necrosis factor alpha-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)]水平。结果治疗后观察组临床总有效率93.33%(28/30)明显高于对照组66.67%(20/30),差异有统计学意义(P<0.05)。治疗后两组患者mMRC、CAT评分均较治疗前降低,差异有统计学意义(P<0.05);且观察组mMRC、CAT评分均明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者PaCO_(2)指标均较治疗前降低,PaO_(2)和SaO_(2)指标均较治疗前升高,差异有统计学意义(P<0.05);且观察组PaCO_(2)指标较对照组明显降低,PaO_(2)和SaO 2指标均较对照组明显升高,差异有统计学意义(P<0.05)。治疗后两组患者PCT、TNF-α、IL-6水平均较治疗前降低,差异有统计学意义(P<0.05),且观察组PCT、TNF-α、IL-6水平均较对照组明显降低,差异有统计学意义(P<0.05)。治疗期间,两组患者均未见与治疗药物相关的不良反应情况发生。结论参芪利水颗粒配合脐灸能够抑制AECOPD炎症反应,改善患者血气分析指标,缓解临床症状。
Objective To investigate the clinical efficacy of Shenqi Lishui granules combined with umbilical moxibustion in treating patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods From August 2022 to August 2023,60 AECOPD patients were selected from the Baoding No.1 Hospital of Traditional Chinese Medicine,Hebei Province.They were randomly divided into a control group and an observation group using a random number table method,with 30 patients in each group.The control group was treated with conventional Western medicine,while the observation group was treated with a combination of umbilical moxibustion and Shenqi Lishui granules based on the control group.After two weeks of treatment,the clinical efficacy and safety were observed and compared between the two groups.The modified British Medical Research Council respiratory questionnaire(mMRC)scores,chronic obstructive pulmonary disease assessment test(CAT)scores,blood gas analysis indicators,and serum inflammatory factors[Procalcitonin(PCT),tumor necrosis factor-alpha(TNF-α),Interleukin-6(IL-6)]levels were assessed between the two groups of patients.Results After treatment,the total clinical efficacy rate in the observation group was 93.33%(28/30),significantly higher than the control group's 66.67%(20/30),with a statistically significance difference(P<0.05).The mMRC and CAT scores of both groups were significantly lower than before treatment,with a statistically significance difference(P<0.05);the observation group's scores were significantly lower than those in the control group,with a statistically significance difference(P<0.05).After treatment,the PaCO_(2) levels in both groups decreased,while the PaO_(2) and SaO_(2) levels increased significantly compared to before treatment,with a statistically significance difference(P<0.05).The observation group's PaCO_(2) levels were significantly lower,and PaO_(2) and SaO_(2) levels were significantly higher than those in the control group,with a statistically significance difference(P<0.05).Serum PCT,TNF-α,and IL-6 levels in both groups decreased significantly compared to before treatment,with a statistically significance difference(P<0.05);the observation group's levels significantly lower than those in the control group,with a statistically significance difference(P<0.05).During the treatment period,no adverse reactions related to the treatment drugs were observed in either group.Conclusion The combination of Shenqi Lishui granules and umbilical moxibustion can inhibit the inflammatory response in AECOPD,improve blood gas analysis indicators,and alleviate clinical symptoms.
