超声检查联合血清VEGF及CA125指标诊断绝经后无症状子宫内膜增厚女性子宫内膜病变的临床价值

Clinical value of ultrasound combined with serum VEGF and CA125 in diagnosing endometrial pathology in postmenopausal women with asymptomatic endometrial thickening

目的探讨超声检查联合血清血管内皮生长因子(VEGF)、癌抗原125(CA125)指标诊断绝经后无症状子宫内膜增厚女性子宫内膜病变的临床价值。方法本研究纳入2022年4月—2023年4月期间驻马店市中心医院收治的绝经后无症状子宫内膜增厚女性子宫内膜病变患者80例作为病例组,另选取同期行健康体检者80例作为对照组,比较两组患者的超声参数、血清CA125和VEGF水平,并通过受试者工作曲线(ROC)分析以上检测指标对子宫内膜病变的诊断效能。结果病例组血清CA125、VEGF水平及最大血流速度(SP)、舒张末期血流速度(ED)等超声参数均较对照组明显偏高,阻力指数(RI)值偏低(均P<0.05)。ROC曲线分析结果显示,超声检查、血清CA125、VEGF水平单独检测及联合检测诊断子宫内膜病变的AUC分别为0.885(95%CI:0.812~0.958)、0.785(95%CI:0.681~0.890)、0.842(95%CI:0.753~0.931)和0.949(95%CI:0.905~0.994),其灵敏度分别为81.25%、62.53%、84.38%和96.88%;特异度为80.09%、82.61%、72.55%和82.52%。结论联合使用超声检查血清CA125和VEGF水平检测可以显著提高子宫内膜病变的诊断准确率,为临床诊断和治疗提供更为可靠的依据。

Objective To explore the clinical value of ultrasound combined with serum vascular endothelial growth factor(VEGF)and carbohydrate antigen 125(CA125)in diagnosing endometrial pathology in postmenopausal women with asymptomatic endometrial thickening.Methods In this study,80 cases of postmenopausal women with asymptomatic endometrial thickening admitted to Zhumadian Central Hospital from April 2022 to April 2023 were included as the case group,and 80 cases of healthy people were selected as the control group.The ultrasonic parameters,serum CA125 and VEGF levels of the two groups were compared,and the diagnostic efficacy of the above indicators for endometrial lesions was analyzed through the receiver's working curve(ROC).Results The levels of serum CA125,VEGF,maximum blood flow velocity(SP),end diastolic blood flow velocity(ED)and other ultrasonic parameters in the case group were significantly higher than those in the control group,and the resistance index(RI)value was lower(all P<0.05).ROC curve analysis showed that AUC of ultrasound,serum CA125 and VEGF levels alone and combined detection in diagnosing endometrial pathology were 0.885(95%CI:0.812-0.958),0.785(95%CI:0.681-0.890),0.842(95%CI:0.753-0.931)and 0.949(95%CI:0.905-0.994),respectively.The specificity was 80.09%,82.61%,72.55% and 82.52%.Conclusion The combined use of ultrasound examination and serum CA125 and VEGF levels can significantly improve the diagnostic accuracy of endometrial lesions and provide a more reliable basis for clinical diagnosis and treatment.
