达格列净联合二甲双胍、阿卡波糖对2型糖尿病患者胰岛功能、糖脂代谢、氧化应激影响

Effects of dapagliflozin combined with metformin and acarbose on islet function,glucose and lipid metabolism,and oxidative stress in T2DM patients

摘要 目的探讨达格列净联合二甲双胍、阿卡波糖对2型糖尿病患者胰岛功能、糖脂代谢、氧化应激的影响。方法选取我院2020年10月至2021年10月收治的58例2型糖尿病患者为研究对象,随机分为观察组与对照组,各29例。观察组患者给与达格列净联合二甲双胍、阿卡波糖治疗,对照组给予二甲双胍联合阿卡波糖治疗。比较两组患者治疗前后胰岛功能、糖脂代谢、氧化应激水平变化情况。结果治疗后两组患者胰岛功能指标中FINS、HOMA-IR均降低,且治疗后观察组FINS(7.13±1.24)mU/L、HOMA-IR(2.24±0.59)均低于对照组FINS(9.55±1.44)mU/L、HOMA-IR(3.18±0.74)(P<0.05);治疗后两组患者糖脂代谢指标中血糖指标FPG、2hPG及血脂指标TG、TC均降低,且治疗后观察组血糖指标FPG(5.75±1.04)mmol/L、2hPG(8.18±1.45)mmol/L及血脂指标TG(1.31±0.33)mmol/L、TC(3.99±0.91)mmol/L均低于对照组FPG(7.48±1.47)mmol/L、2hPG(10.11±1.59)mmol/L及血脂指标TG(1.78±0.47)mmol/L、TC(4.75±1.01)mmol/L(P<0.05);治疗后两组患者氧化应激指标中氧化产物含量MDA均降低,抗氧化酶含量SOD均升高,且治疗后观察组MDA(8.01±1.64)nmol/mL低于对照组MDA(10.38±2.43)nmol/mL,SOD(95.33±10.49)U/L高于对照组SOD(82.74±11.58)U/L(P<0.05)。结论达格列净联合二甲双胍、阿卡波糖能有效调节2型糖尿病患者胰岛功能,降低血糖、血脂水平,减轻氧化应激反应,值得推广。 Objective To investigate the effects of dapagliflozin combined with metformin and acarbose on islet function,glucose and lipid metabolism,and oxidative stress in patients with type 2 diabetes mellitus(T2DM).Methods A total of 58 patients with T2DM admitted to our hospital from October 2020 to October 2021 were selected and randomly divided into the observation group and the control group,with 29 cases in each group.Patients in the observation group were treated with dapagliflozin combined with metformin and acarbose,and those in the control group were treated with metformin combined with acarbose.The changes of islet function,glucose and lipid metabolism,and oxidative stress levels before and after treatment were compared between the two groups.Results After treatment,FINS and HOMA-IR in islet function indicators were decreased in both groups,and FINS(7.13±1.24)mU/L and HOMA-IR(2.24±0.59)in the observation group were lower than those[FINS(9.55±1.44)mU/L and HOMA-IR(3.18±0.74)]in the control group after treatment(P<0.05).After treatment,FPG and 2hPG in glucose and lipid metabolism indicators and TG and TC in blood lipid indicators were decreased in both groups,and after treatment,the blood glucose indicators FPG(5.75±1.04)mmol/L,2hPG(8.18±1.45)mmol/L and blood lipid indicators TG(1.31±0.33)mmol/L,TC(3.99±0.91)mmol/L in the observation group were lower than those in the control group[(7.48±1.47)mmol/L,(10.11±1.59)mmol/L and(1.78±0.47)mmol/L,(4.75±1.01)mmol/L](P<0.05).After treatment,the oxidative product content MDA decreased and the antioxidant enzyme content SOD increased in the oxidative stress indicators of the two groups,and the MDA(8.01±1.64)nmol/mL in the observation group was lower than that in the control group[(10.38±2.43)nmol/mL],but SOD(95.33±10.49)U/L was higher than in the control group[(82.74±11.58)U/L](P<0.05).Conclusion Dapagliflozin combined with metformin and acarbose can effectively regulate islet function,reduce blood glucose and blood lipid levels and reduce oxidative stress in patients with type 2 diabetes mellitus.It is worth popularizing.
