支气管肺发育不良早产儿血清HO-1、NF-κB水平及其与预后的关系

摘要 目的:探讨支气管肺发育不良(BPD)早产儿血清血红素加氧酶-1(HO-1)、核转录因子κB(NF-κB)的水平及其与预后的关系。方法:选择2020年1月至2023年12月郑州市妇幼保健院收治的60例BPD早产儿(BPD组)与85例无BPD早产儿(n-BPD组)作为研究对象,检测其出生后24 h、3 d、7 d的血清HO-1、NF-κB水平,并分析其对BPD的预测价值。根据预后将BPD早产儿分为预后良好组(23例)与预后不良组(37例)。检测其出生后24 h、3 d、7 d的血清HO-1、NF-κB水平及其对BPD预后的预测价值。结果:BPD组与n-BPD组出生后24 h、3 d、7 d的血清HO-1水平均持续升高,但BPD组出生后3 d、7 d HO-1水平低于n-BPD组,差异均具有统计学意义(P<0.05)。BPD组出生后24 h、3 d、7 d的血清NF-κB水平持续升高,且均高于n-BPD组,差异均具有统计学意义(P<0.05)。受试者工作特征(ROC)曲线分析显示,出生后7 d血清HO-1、NF-κB水平均对BPD的发生具有中等预测价值。预后良好组与预后不良组出生后24 h、3 d、7 d的血清HO-1、NF-κB水平均持续升高,预后不良组出生后3 d、7 d血清HO-1水平低于预后良好组,NF-κB水平高于预后良好组,差异均具有统计学意义(P<0.05)。ROC曲线分析显示,出生后7 d血清NF-κB水平对BPD预后具有中等预测价值。结论:血清HO-1、NF-κB水平与早产儿发生BPD有关,对预测BPD的发生具有潜能,在预测BPD预后方面,NF-κB优于HO-1。 Objective To investigate the serum levels of heme oxygenase-1(HO-1)and nuclear transcription factorκB(NF-κB)in premature infants with bronchopulmonary dysplasia(BPD)and their relationship with prognosis.Methods Sixty premature infants with BPD(BPD group)and 85 premature infants without BPD(n-BPD group)admitted to Women&Infants Hospital of Zhengzhoul from January 2020 to December 2023 were selected as the study objects.The serum levels of HO-1 and NF-κB at 24 hours,3 days,and 7 days after birth were measured,and their predictive value for BPD was analyzed.According to the prognosis,premature infants with BPD were divided into a good prognosis group(23 cases)and a poor prognosis group(37 cases).The serum levels of HO-1 and NF-κB at 24 hours,3 days,and 7 days after birth were detected and their predictive value for the prognosis of BPD was analyzed.Results The serum level of HO-1 in the BPD group and the n-BPD group continued to increase at 24 hours,3 days,and 7 days after birth,but the serum level of HO-1 in the BPD group was lower than that in the n-BPD group at 3 and 7 days after birth,and the differences were statistically significant(P<0.05).The serum levels of NF-κB in the BPD group continued to increase at 24 hours,3 days,and 7 days after birth,and were all higher than those in the n-BPD group,with statistically significant differences(P<0.05).The receiver operating characteristic(ROC)curve analysis showed that the serum levels of HO-1 and NF-κB at 7 days after birth had moderate predictive value for the occurrence of BPD.The serum levels of HO-1 and NF-κB continued to increase at 24 hours,3 days,and 7 days after birth in the good prognosis group and the poor prognosis group.The serum level of HO-1 in the poor prognosis group was lower than that in the good prognosis group at 3 days and 7 days after birth,while the NF-κB level was higher than that in the good prognosis group,and the differences were statistically significant(P<0.05).The ROC curve analysis showed that the serum level of NF-κB at 7 days after birth had moderate predictive value for the prognosis of BPD.Conclusion The serum levels of HO-1 and NF-κB are associated with the occurrence of BPD in premature infants and have the potential to predict the occurrence of BPD.NF-κB is superior to HO-1 in predicting the prognosis of BPD.
