纺锤体观测仪在高龄患者卵胞浆内单精子显微注射术中的应用价值

Application Value of Spindle Observation Instrument in Elderly Patients with Intracytoplasmic Sperm Mcroinjection

目的:探讨纺锤体观测仪在高龄患者卵胞浆内单精子显微注射术(ICSI)中的临床价值。方法:选取2022年1月至2023年6月九江市妇幼保健院辅助生殖中心收治的高龄女性患者ICSI共121例,根据ICSI助孕方法的不同分为对照组(49例)和观察组(72例)。对照组采用传统方法(ICSI进针位置与第一极体呈90°夹角),观察组使用纺锤体观测仪引导ICSI进针位置。比较两组患者获卵平均数、MⅡ卵平均数、胚胎各项指标。结果:两组患者获卵平均数、MⅡ卵平均数比较,差异无统计学意义(P>0.05)。观察组患者2PN受精率、2PN受精卵裂胚胎率、优质胚胎率、囊胚形成率、临床妊娠率均高于对照组,差异具有统计学意义(P<0.05)。结论:在高龄患者中,使用纺锤体观测仪下进行ICSI助孕,其胚胎各项指标要优于传统ICSI助孕。

Objective To investigate the clinical value of spindle observation instrument in intracytoplasmic sperm microinjection(ICSI)in elderly patients.Methods A total of 121 elderly women patients with ICSI admitted to the Assisted Reproduction Center of Jiujiang Maternal and Child Health Hospital from January 2022 to June 2023 were selected and divided into a control group(49 cases)and an observation group(72 cases)according to different methods of ICSI assisted pregnancy.The traditional method was used in the control group(the ICSI insertion position was at 90°Angle from the first pole body),and the observation group was guided by the spindle observation instrument.The mean number of eggs retrieved,the mean number of MⅡeggs and the indexes of embryos were compared between the two groups.Results There was no significant difference in the mean number of eggs retrieved and the mean number of MⅡeggs between the two groups(P>0.05).The 2PN fertilization rate,2PN fertilized cleavage embryo rate,high-quality embryo rate,blastocyst formation rate and clinical pregnancy rate in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion In elderly patients,the embryo indicators of ICSI assisted pregnancy with spindle observation instrument were better than those of traditional ICSI assisted pregnancy.
