血清sEng、PLGF、LDH水平与妊娠期高血压患者病情的相关性及其诊断价值

Correlation and diagnostic value of serum sEng,PLGF,LDH levels in hypertensive patients during pregnancy

目的探讨血清可溶性内皮因子(sEng)、胎盘生长因子(PLGF)、乳酸脱氢酶(LDH)水平与妊娠期高血压(HDCP)患者病情的相关性及其诊断价值。方法选取2021年1月至2022年5月收治的100例HDCP患者为观察组,另选取同期100例健康孕妇为对照组。比较两组及不同病情严重程度患者入院时血清sEng、PLGF、LDH水平,分析入院时各指标水平与病情严重程度的相关性,logistic回归分析发生HDCP的影响因素,ROC分析入院时各指标联合检测对HDCP的诊断价值。结果入院时血清sEng、LDH水平比较:观察组>对照组,HDCP<子痫前期<重度子痫前期,PLGF水平比较:观察组<对照组,HDCP>子痫前期>重度子痫前期(P<0.05);入院时血清sEng、LDH水平与HDCP患者病情严重程度呈正相关,PLGF水平与患者病情严重程度呈负相关(P<0.001);入院时血清sEng、LDH水平为发生HDCP的危险因素,PLGF水平为保护因素,各指标联合诊断HDCP的AUC为0.815。结论血清sEng、PLGF、LDH水平与HDCP患者病情严重程度密切相关,联合检测对HDCP具有一定诊断价值,可为临床早期诊断疾病、评估病情严重程度提供可靠参考。

Objective To investigate correlation between serum soluble endoglin(sEng),placenta growth factor(PLGF)and lactate dehydrogenase(LDH)levels and hypertensive disorders complicating pregnancy(HDCP)and its diagnostic value.Methods 100 HDCP patients admitted to hospital from January 2021 to May 2022 were selected as observation group,and 100 healthy pregnant women during the same period were selected as control group.The levels of serum sEng,PLGF,and LDH in patients with different severity of the disease at the time of admission were compared,and correlation between the levels of these indicators at admission and the severity of the disease was analyzed.The influencing factors of HDCP were analyzed by logistic regression,and the diagnostic value of combined detection of each index at admission was analyzed by ROC.Results At admission,serum sEng and LDH levels were compared:observation group>control group,HDCP<preeclampsia<severe preeclampsia;PLGF levels were compared:observation group<control group,HDCP>preeclampsia>severe preeclampsia(P<0.05);serum sEng and LDH levels were positively correlated with the severity of HDCP patients at admission,while PLGF levels were negatively correlated with the severity of HDCP patients(P<0.001).At admission,serum sEng and LDH levels were risk factors for HDCP,and PLGF levels were protective factors.The AUC for combined diagnosis of HDCP was 0.815.Conclusion The levels of serum sEng,PLGF,and LDH are closely related to the severity of HDCP,and combined detection has certain diagnostic value for HDCP,which can provide a reliable reference for early clinical diagnosis and assessment of disease severity.
