经皮椎体成形术对老年骨质疏松性椎体压缩性骨折患者的疗效及生活质量的影响

Effect of Percutaneous Vertebroplasty on Senile Osteoporotic Vertebral Compression Fractures and Quality of Life

目的:分析经皮椎体成形术对老年骨质疏松性椎体压缩性骨折患者的疗效及生活质量的影响。方法:选取2021年6月—2023年2月信阳市人民医院骨科收治的100例老年骨质疏松性椎体压缩性骨折患者作为研究对象,分别实施保守治疗(对照组,n=45)和保守治疗+经皮椎体成形术治疗(观察组,n=55)。对比两组患者治疗前与治疗后3个月的伤椎功能评分、腰部疼痛评分及椎体结构指标、骨密度与骨代谢指标、生活质量评分情况。结果:治疗后,观察组患者的伤椎功能评分、疼痛评分均较对照组明显降低,椎体前缘高度、椎体中线高度均较对照组升高,Cobb角较对照组缩小,差异有统计学意义(t=9.499、12.204、6.939、7.358、14.776,P<0.05);治疗后,观察组患者的骨密度、血清骨钙素(BGP)、骨碱性磷酸酶(BALP)均较对照组升高,差异有统计学意义(t=9.020、6.574、6.154,P<0.05);治疗后,观察组患者的躯体疼痛、健康状况、生理机能、生理职能4个维度评分均较对照组升高,差异有统计学意义(t=7.588、5.952、5.873、6.877,P<0.05)。结论:经皮椎体成形术治疗老年骨质疏松性椎体压缩性骨折效果较好,可有效改善患者的伤椎功能、椎体结构指标、骨密度与骨代谢指标,缓解患者疼痛,提高患者的生活质量。

Objective:To analyze the effect of percutaneous vertebroplasty on senile osteoporotic vertebral compression fracture and its influence on patients’quality of life.Methods:A total of 100 senile patients with osteoporotic vertebral compression fractures were selected from June 2021 to February 2023.Patients were divided into two groups on average,and conservative treatment(control group,n=45)and conservative treatment+percutaneous vertebroplasty(observation group,n=55)were respectively conducted.The injured vertebral function score,lumbar pain score,vertebral structure index,bone density and bone metabolism index,and quality of life score were compared between the two groups before treatment and 3 months after treatment.Results:After treatment,the functional scores and pain scores of injured vertebrae in observation group were significantly lower than those in control group,the height of anterior vertebral edge and midline of vertebral body were higher than those in control group,and the Cobb Angle was smaller than that in control group(t=9.499,12.204,6.939,7.358,14.776;P<0.05).After treatment,bone mineral density,BGP and BALP in observation group were higher than those in control group(t=9.020,6.574,6.154;P<0.05).After treatment,the scores of physical pain,health status,physiological function and physiological function in observation group were higher than those in control group(t=7.588,5.952,5.873,6.877;P<0.05).Conclusion:Percutaneous vertebroplasty is effective in the treatment of osteoporotic vertebral compression fracture in elderly patients,which can effectively improve the injured vertebral function,vertebral structure index,bone mineral density and bone metabolism index;relieve the pain of patients;and improve the quality of life of patients.
