参芪益母膏治疗剖宫产术后子宫复旧不良(气虚血瘀型)的临床观察

Clinical Observation of Shenqi Yimu Ointment in the Treatment of Subinvolution of Uterus After Cesarean of Qi Deficiency and Blood Stasis Type

目的:观察参芪益母膏对剖宫产术后子宫复旧不良(气虚血瘀型)的临床疗效。方法:选取2022年1月—2023年1月重庆市涪陵区中医院81例剖宫产术后子宫复旧不良患者纳入研究,将其随机分为观察组40例,对照组41例。对照组患者给予缩宫素治疗,观察组在此基础上服用参芪益母膏。治疗7 d,观察两组患者临床疗效、血性恶露持续时间、腹痛持续时间、子宫下降高度及不良反应。结果:观察组患者治疗总有效率为92.50%,高于对照组的75.61%,差异有统计学意义(χ^(2)=4.278,P<0.05)。观察组患者血性恶露持续时间、腹痛持续时间较对照组明显缩短,差异有统计学意义(t=4.624、6.475,P<0.05)。两组患者用药后在第1 d、第3 d子宫底下降高度比较,差异无统计学意义(P>0.05);在治疗第5 d,观察组患者子宫底下降高度明显低于对照组,差异有统计学意义(t=-2.630,P<0.05)。临床观察期间,两组患者未出现明显不良反应。结论:与单纯使用缩宫素相比,参芪益母膏的联用能提高临床疗效、缩短血性恶露持续时间和腹痛持续时间以及加快子宫底下降速度,从而明显促进产妇剖宫产后子宫复旧。

Objective:To observe the clinical effect of the Shenqi yimu ointment in the treatment of subinvolution of uterus after cesarean.Methods:81 patients with subinvolution of uterus after cesarean section were randomly divided into observation group(n=40)and control group(n=41).The control group was treated with oxytocin,and the observation group was given Shenqi yimu ointment on the basis of the control group.The therapeutic effects,symptoms improvement,descending height of uterus and adverse reaction were observed.Results:The total effective rate of the observation group was 92.50%,which was significantly higher than 75.61%of control group,with statistically significant difference(χ^(2)=4.278,P<0.05).The duration of lochia rubra and abdominal pain in the observation group were significantly shorter than those in the control group(t=4.624,6.475;P<0.05).There was no significant difference in descending height of uterine floor between the two groups on the 1st and 3rd day after treatment(P>0.05).On the 5th day of treatment,the descending height of uterus in the observation group was significantly lower than that in the control group(t=-2.630,P<0.05).No obvious adverse reaction occured in the two groups during the treatment.Conclusion:Compared with the simple use of oxytocin,the combination of Shenqi Yimu paste can improve the clinical effect,shorten the duration of lochia haemorrhagica and abdominal pain,and accelerate the descent of the uterine floor,which can significantly promote the uterine rehabilitation after cesarean section.
