加味桃红四物汤在老年股骨粗隆间骨折术后患者中的应用效果及对凝血功能的影响

Application Effect of Jiawei Taohong Siwu Decoction in Elderly Patients with Intertrochanteric Fracture of Femur after Surgery and Its Influence on Coagulation Function

目的:探讨老年股骨粗隆间骨折(IFOF)患者术后应用加味桃红四物汤的效果及对凝血功能的影响。方法:选取2022年10月—2023年10月南昌市洪都中医院收治的老年IFOF患者82例,以随机数字表法分为对照组(n=41)与研究组(n=41)。两组均接受股骨近端防旋髓内钉(PFNA)内固定术治疗,对照组术后给予常规对症治疗,研究组术后在对照组基础上加用加味桃红四物汤治疗。比较两组临床疗效、术后恢复情况、髋关节功能、凝血功能及炎症因子。结果:研究组总有效率[97.56%(40/41)]较对照组[80.49%(33/41)]高,差异有统计学意义(P<0.05);研究组患肢肿胀消退时间、骨痂形成时间、开始负重时间、骨折愈合时间均较对照组短,差异均有统计学意义(P<0.05);治疗后两组Harris评分均升高,研究组较对照组高,差异均有统计学意义(P<0.05);治疗后两组血小板聚集率(PAgR)、D-二聚体(D-D)、纤维蛋白原(FIB)水平均下降,研究组均较对照组低,差异均有统计学意义(P<0.05);治疗后两组白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平均下降,研究组均较对照组低,差异均有统计学意义(P<0.05)。结论:老年IFOF患者术后应用加味桃红四物汤治疗,能够提高临床疗效,促进患者术后恢复,改善髋关节功能、凝血功能,调节炎症因子水平。

Objective:To explore the effect of Jiawei Taohong Siwu Decoction in elderly patients with intertrochanteric fracture of femur(IFOF)after surgery and its influence on coagulation function.Method:A total of 82 elderly patients with IFOF admitted to Nanchang Hongdu Hospital of TCM from October 2022 to October 2023 were selected and divided into control group(n=41)and study group(n=41)by random number table method.Both groups received proximal femoral nail anti-rotation(PFNA)internal fixation treatment,the control group was given conventional symptomatic treatment after surgery,and the study group was treated with Jiawei Taohong Siwu Decoction after surgery on the basis of the control group.The clinical efficacy,postoperative recovery,hip joint function,coagulation function and inflammatory factors were compared between the two groups.Result:The total effective rate of the study group[97.56%(40/41)]was higher than that of the control group[80.49%(33/41)],the difference was statistically significant(P<0.05).The time of swelling subsided of affected limb,bone callus formation,weight bearing and fracture healing in the study group were shorter than those in the control group,the differences were statistically significant(P<0.05).After treatment,the Harris scores of both groups were increased,and that in the study group was higher than that in the control group,the differences were statistically significant(P<0.05).After treatment,the platelet aggregation rate(PAgR),D-dimer(D-D)and fibrinogen(FIB)levels of both groups were decreased,and those in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-1(IL-1),interleukin-6(IL-6)and interleukin-8(IL-8)in both groups were decreased,and those in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The application of Jiawei Taohong Siwu Decoction in the elderly patients with IFOF after surgery can improve the clinical effect,promote the postoperative recovery of patients,improve the hip joint function and coagulation function,and regulate the levels of inflammatory factors.
