经尿道前列腺等离子电切术对良性前列腺增生患者前列腺症状及性功能的影响

Effect of Trans Urethral Plasma Kinetic Resection of the Prostate on Prostatic Symptoms and Sexual Function in Patients with Benign Prostatic Hyperplasia

目的:分析经尿道前列腺等离子电切术(TUPKRP)对良性前列腺增生(BPH)患者前列腺症状及性功能的影响。方法:前瞻性选取2020年1月—2023年1月盐城市大丰人民医院泌尿外科收治的82例有手术指征的BPH患者作为研究对象。随机分为TURP组[行经尿道前列腺切除术(TURP)治疗]与TUPKRP组(TUPKRP治疗),各41例。比较两组围手术期指标、手术并发症、前列腺症状[残余尿量(PVR)、最大尿流速(Qmax)、国际前列腺症状评分(IPSS)评分]、性能力[国际勃起功能简表指数-5(IIEF-5)评分]。结果:TUPKRP组手术时间、住院时间均短于TURP组,术中出血量少于TURP组,切除组织重量高于TURP组,差异均有统计学意义(P<0.05);两组导尿时间比较,差异无统计学意义(P>0.05)。TUPKRP组术后并发症发生率低于TURP组,差异有统计学意义(P<0.05)。术后6个月,TUPKRP组PVR、IPSS评分均低于TURP组,Qmax、IIEF-5评分均高于TURP组,差异均有统计学意义(P<0.05)。结论:TUPKRP具有手术时间短、创伤小、更多切除前列腺增生组织等优势,有利于改善BPH患者前列腺症状,降低并发症发生率,促进性能力恢复。

Objective:To analyze the effects of trans urethral plasma kinetic resection of the prostate(TUPKRP)on prostatic symptoms and sexual function in patients with benign prostatic hyperplasia(BPH).Method:A total of 82 BPH patients with surgical indications admitted to the Department of Urology of Dafeng People's Hospital of Yancheng from January 2020 to January 2023 were prospectively selected as the study objects.They were randomly divided into TURP group[treated with trans urethral resection of the prostate(TURP)]and TUPKRP group(treated with TUPKRP),with 41 cases in each group.Perioperative indexes,surgical complications,prostate symptoms[postvoid residual urine(PVR),maximum urine flow rate(Qmax),international prostate symptom score(IPSS)]and sexual function[international index of erectile function-5(IIEF-5)]were compared between the two groups.Result:The operation time and hospital stay in TUPKRP group were shorter than those in TURP group,the intraoperative blood loss was less than that in TURP group,and the weight of excised tissue was higher than that in TURP group,the differences were statistically significant(P<0.05).There was no significant difference in catheterization time between the two groups(P>0.05).The incidence of postoperative complications in TUPKRP group was lower than that in TURP group,the difference was statistically significant(P<0.05).Six months after surgery,PVR and IPSS score in TUPKRP group were lower than those in TURP group,and Qmax and IIEF-5 score were higher than those in TURP group,with statistical significance(P<0.05).Conclusion:TUPKRP has the advantages of short operation time,less trauma,and more resection of prostate hyperplasia tissue,which is conducive to improving prostate symptoms in BPH patients,reducing the incidence of complications,and promoting the recovery of sexual capacity.
