负压一次性输尿管软镜碎石术对上尿路结石患者结石清除率及炎症因子水平的影响

Effect of Negative Pressure Disposable Ureteroscopy Lithotripsy on Stone Clearance Rate and Inflammatory Factors Levels in Patients with Upper Urinary Tract Stones

目的:探讨负压一次性输尿管软镜碎石术对上尿路结石患者结石清除率及炎症因子水平的影响。方法:选取2021年10月—2023年10月南京医科大学附属苏州医院泌尿外科收治的98例上尿路结石患者,采用摸球法分为两组。对照组(n=49)采用一次性输尿管软镜碎石术治疗,观察组(n=49)采用负压一次性输尿管软镜碎石术治疗。比较两组手术相关指标、炎症因子水平及术后并发症发生率。结果:观察组手术时间短于对照组,治疗费用少于对照组,差异均有统计学意义(P<0.05);观察组术后3 d结石清除率为87.75%,高于对照组的69.69%,差异有统计学意义(P<0.05);观察组术后降钙素原(PCT)、白细胞介素-1β(IL-1β)、前列腺素E2(PGE2)和C反应蛋白(CRP)水平均低于对照组,差异均有统计学意义(P<0.05);观察组术后并发症发生率低于对照组(P<0.05)。结论:负压一次性输尿管软镜碎石术应用于上尿路结石,可提高近期结石清除率,缩短手术时间,引起的炎症反应较轻,且可减少并发症发生。

Objective:To investigate the effect of negative pressure disposable ureteroscopy lithotripsy on stone clearance rate and inflammatory factors levels in patients with upper urinary tract stones.Method:A total of 98 patients with upper urinary tract stones admitted to Department of Urology,the Affiliated Suzhou Hospital of Nanjing Medical University from October 2021 to October 2023 were selected and divided into two groups by ball-touching method.The control group(n=49)was treated with disposable ureteroscopy lithotripsy,and the observation group(n=49)treated with negative pressure disposable ureteroscopy lithotripsy.Operation-related indexes,inflammatory factors levels and incidence of postoperative complications were compared between the two groups.Result:The operation time of observation group was shorter than that of control group,and the treatment cost was less than that of control group,the differences were statistically significant(P<0.05).The stone clearance rate 3 days after operation in the observation group was 87.75%,which was higher than 69.69%in the control group,the difference was statistically significant(P<0.05).The levels of procalcitonin(PCT),interleukin-1β(IL-1β),prostaglandin E2(PGE2)and C reactive protein(CRP)in observation group were lower than those in control group,the differences were statistically significant(P<0.05).The incidence of postoperative complications in observation group was lower than that in control group(P<0.05).Conclusion:The application of negative pressure disposable ureteroscopy lithotripsy in upper urinary tract stones can improve the stone clearance rate,shorten the operation time,cause less inflammatory reaction,and reduce the occurrence of complications.
