超声指导下可视喉镜联合床头抬高30°在肥胖患者气管插管中的应用效果

摘要 目的探讨超声指导下可视喉镜联合床头抬高30°在肥胖患者气管插管中的应用效果。方法选择2020年3月7日至2021年10月20日东阿县人民医院重症医学科收治的84例肥胖气管插管困难患者,随机将其分为对照组(34例)和观察组(50例)。对照组采用常规气管插管方法,观察组给予规范化超声指导下可视喉镜联合床头抬高30°气管插管。比较两组的应用效果。结果观察组的一次性插管成功率明显高于对照组,二次插管率明显低于对照组,气管插管时间明显短于对照组,差异具有统计学意义(P<0.05)。观察组的不良事件总发生率低于对照组,差异具有统计学意义(P<0.05)。观察组的耐受占比明显高于对照组,差异具有统计学意义(P<0.05)。结论超声指导下可视喉镜联合床头抬高30°在肥胖患者气管插管中的应用价值较高,优化了插管流程,提高了一次性插管成功率,减少了不良事件的发生,也减轻了患者的痛苦,便于临床开展实施。 Objective To explore the application effect of visual laryngoscope under the guidance of ultrasound combined with bedside elevation 30°in tracheal intubation of obese patients.Methods A total of 84 obese patients with difficult tracheal intubation admitted in the critical care medicine department of the People's Hospital of Dong'e County from March 7,2020 to October 20,2021 were selected and randomly divided into control group(34 cases)and observation group(50 cases).The control group was given conventional tracheal intubation method,and the observation group was given visual laryngoscope under the standardized guidance of ultrasound combined with bedside elevation of 30°in tracheal intubation.The application effects of the two groups were compared.Results The one-time intubation success rate of the observation group was significantly higher than that of the control group,the secondary intubation rate was significantly lower than that of the control group,the tracheal intubation time was significantly shorter than that of the control group,and the differences were statistically significant(P<0.05).The total incidence of adverse events in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The proportion of tolerance in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The visual laryngoscope under the guidance of ultrasound combined with bedside elevation of 30°in tracheal intubation of obese patients has high application value,which optimizes the intubation process,improves the success rate of one-time intubation,reduces the occurrence of adverse events,relieves the pain of patients,and facilitates clinical implementation.
