清络散外敷治疗急性痛风性关节炎临床观察

Clinical Observation on the Treatment of Acute Gouty Arthritis with External Application of Qingluo Powder

目的:观察清络散外敷联合塞来昔布治疗急性痛风性关节炎(acute gouty arthritis,AGA)的疗效。方法:将60例AGA患者随机分为对照组和治疗组,每组各30例。对照组予口服塞来昔布(0.2g/次,2次/d,连服3d后改为0.2g/次,1次/d,续服4d);治疗组在对照组的基础上外敷清络散,1次/d,每次6~8h,充分覆盖患处关节。2组均治疗1周。观察2组治疗前后中医证候积分,VAS评分,并检测血沉(ESR)、C反应蛋白(CRP)、血清尿酸(SUA)水平,记录疼痛缓解时间、红肿消除时间。结果:治疗组有效率高于对照组;2组中医证候积分、VAS评分以及ESR、CRP、SUA均有所降低;治疗组在疼痛缓解时间、红肿消除时间表现更快,差异均有统计学意义。结论:清络散外敷联合塞来昔布治疗湿热蕴结型急性痛风性关节炎比单纯塞来昔布方案能更迅速缓解临床症状,改善生活质量,值得临床推广。

Objective:To observe the efficacy of Qingluo powder combined with celecoxib in the treatment of acute gouty arthritis(AGA).Methods:Sixty patients with AGA were randomly divided into a study group and a control group,with 30 cases in each group.The control group was given oral celecoxib(0.2 g/time,2 times/d,for 3 days,then changed to 0.2 g/time,1 time/d,for 4 days);the treatment group was given Qingluo powder on the basis of the control group,1 time/d,6-8 hours each time,fully covering the affected joints.Both groups were treated for 1 week.The TCM syndrome scores and VAS scores were observed before and after treatment in the two groups,and the levels of erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),and serum uric acid(SUA)were detected,and the pain relief time and redness and swelling elimination time were recorded.Results:The effective rate of the treatment group was higher than that of the control group;the TCM syndrome scores,VAS scores,ESR,CRP,and SUA of the two groups were reduced;the treatment group showed faster pain relief time and redness and swelling elimination time,and the differences were statistically significant.Conclusion:Qingluo powder combined with celecoxib can relieve clinical symptoms more quickly and improve the quality of life in the treatment of acute gouty arthritis of damp-heat accumulation type than celecoxib alone,which is worthy of clinical promotion.
