开放手术与腹腔镜结肠癌根治术治疗老年结肠癌的效果对比

Comparison of Effect of Open Surgery and Laparoscopic Radical Resection of Colon Cancer in Elderly Patients

目的:探究开放手术与腹腔镜结肠癌根治术治疗老年结肠癌的临床效果。方法:选取2020年6月—2023年5月常州市第七人民医院收治的60例老年结肠癌患者为研究对象,随机分为对照组与观察组,各30例。对照组接受开放手术,观察组接受腹腔镜结肠癌根治术。比较两组临床疗效、胃肠功能指标、康复指标以及术后1年复发率、转移率与生存率。结果:两组治疗总有效率比较,差异无统计学意义(P>0.05)。治疗后,两组胃动素、胃泌素水平降低,但观察组高于对照组(P<0.05)。观察组术后肠道功能恢复时间、住院时间短于对照组,术中出血量、术后镇痛次数少于对照组(P<0.001)。两组术后1年复发率与转移率比较,差异无统计学意义(P>0.05);观察组术后1年生存率高于对照组(P=0.028)。结论:开放手术与腹腔镜结肠癌根治术治疗老年结肠癌的转移率未见明显差异,但腹腔镜结肠癌根治术远期生存率更高,且对患者胃肠功能的影响较小,创伤较轻,术后康复更快。

Objective:To explore the clinical effect of open surgery and laparoscopic radical resection of colon cancer in elderly patients.Methods:Sixty elderly patients with colon cancer treated in Changzhou Seventh People's Hospital from June 2020 to May 2023 were selected as the study objects,and randomly divided into control group and observation group,with thirty cases in each group.The control group received open surgery,and the observation group received laparoscopic radical resection of colon cancer.The clinical efficacy,gastrointestinal function index,rehabilitation index,recurrence rate,metastasis rate and survival rate were compared between the two groups.Results:There was no significant difference in the total effective rate between the two groups(P>0.05).After treatment,the levels of motilin and gastrin in two groups decreased,but the observation group was higher than the control group(P<0.05).The postoperative intestinal function recovery time and hospital stay in the observation group were shorter than those in the control group,and the amount of intraoperative blood loss and postoperative analgesia were shorter than those in the control group(P<0.001).There was no significant difference in recurrence rate and metastasis rate between the two groups(P>0.05);the 1-year survival rate of observation group was higher than that of control group(P=0.028).Conclusion:There is no significant difference in the metastasis rate between open surgery and laparoscopic radical colon cancer in the treatment of elderly colon cancer,but laparoscopic radical colon cancer has a higher long-term survival rate,less impact on the gastrointestinal function of patients,less trauma,and faster postoperative recovery.
