摘要
目的 评估更年期激素治疗方案(MHT)和乳腺癌(BC)患病的相关性。方法 以2020年3月-2022年12月河南省第二人民医院就诊的BC患者作为观察组,以健康体检人群作为对照组,对比两组研究对象的一般资料和MHT。多因素Logisitic回归分析探讨MHT与患者BC患病的相关性,并构建BC患病的预测模型。结果 (1)多因素Logistic回归分析显示体质量指数(BMI)、高血压、糖尿病、BC家族史、孕次、食用油炸食品≥3次/W、食用腌制品≥3次/W、吸烟、使用合成孕激素等,均是BC患病的影响因素(P<0.05)。其中孕次是保护性因素(OR<1)。(2)经ROC分析构建的BC患病Log P预测模型,其AUC为0.863(95%CI:0.791~0.911),显示预测效能较高。结论 合成孕激素是与BC患病相关的MHT处方药物,微粒化孕酮可能是更安全的孕激素,Log P预测模型对更年期女性BC患病的预测效能较好。
Objective To evaluate the correlation between menopausal hormone therapy(MHT)regimens and breast cancer(BC)risk.Methods BC patients treated at Henan Second Provincial People's Hospital from March 2020 to December 2022 were selected as the observation group,while healthy individuals undergoing physical examinations served as the control group.General information and MHT regimens were compared between the two groups.Multivariate Logistic regression analysis was conducted to explore the correlation between MHT and BC risk,and a predictive model for BC development was constructed.Results(1)Multivariate Logistic regression analysis revealed that body mass index(BMI),hypertension,diabetes,BC family history,number of pregnancies,consumption of fried foods≥3 times/week,consumption of pickled foods≥3 times/week,smoking,and synthetic progestogens were factors influencing BC risk(P<0.05).Among them,the number of pregnancies was a protective factor(OR<1).(2)The BC risk prediction model constructed through ROC analysis(including the use of synthetic progestogens)had an AUC of 0.863(95%CI:0.791-0.911),indicating a high predictive performance.Conclusion Synthetic progestogens are MHT prescription drugs associated with BC risk,while micronized progesterone may be a safer option.The predictive model exhibits good predictive performance for BC risk among menopausal women.
