以循证为支持的集束化护理对ICU血液透析患者相关并发症预防价值

The Value of Evidence-Based Cluster Nursing in Prevention of Complications in ICU Patients with Hemodialysis

目的对重症监护病房(ICU)血液透析患者实施以循证为支持的集束化护理,旨在探究该护理模式对相关并发症的预防价值。方法回顾性分析90例在2021年5月至2023年5月就诊于我院ICU的血液透析患者的资料,参照不同护理方式分组,观察组47例,对照组43例。对照组的护理方法为常规护理,观察组的护理方法为以循证为支持的集束化护理。比较两组患者血液透析相关并发症发生情况、导管留置时长、ICU住院时间、生活质量。结果观察组血液透析相关并发症(透析低血糖、凝血、失衡综合征)总发生率低于对照组(P<0.05)。观察组导管留置时长、ICU住院时间均短于对照组(P<0.05)。两组护理前透析患者生活质量量表(KDQ)评分对比差异无统计学意义(P>0.05);将两组护理后的KDQ评分与护理前比较,具有统计上的差异性(P<0.05),且观察组较对照组高(P<0.05)。结论对ICU血液透析患者实施以循证为支持的集束化护理可以预防发生血液透析相关并发症,缩短导管留置时间和ICU住院时间,提高生活质量。

Objective To implement evidence-based bundled nursing for hemodialysis patients in the intensive care unit(ICU),with the aim of exploring the preventive value of this nursing model for related complications.Methods A retrospective analysis was conducted on the data of 90 hemodialysis patients who visited our ICU from May 2021 to May 2023.The patients were divided into two groups based on different nursing methods:an observation group of 47 cases and a control group of 43 cases.The nursing method for the control group was routine nursing,while the nursing method for the observation group was evidence-based bundled nursing.Compare the incidence of complications related to hemodialysis,duration of catheterization,ICU stay,and quality of life between two groups of patients.Results The total incidence of complications related to hemodialysis(dialysis hypoglycemia,coagulation,and imbalance syndrome)in the observation group was lower than that in the control group(P<0.05).The duration of catheter retention and ICU stay in the observation group were shorter than those in the control group(P<0.05).There was no statistically significant difference in the quality of life scale(KDQ)scores between the two groups of dialysis patients before nursing(P>0.05).The KDQ score of the two groups after nursing was statistically different from that before nursing(P<0.05),and the observation group was higher than the control group(P<0.05).Conclusions Implementing evidence-based bundled care for ICU hemodialysis patients can prevent complications related to hemodialysis,shorten catheter retention time and ICU hospitalization time,and improve quality of life.
