早期非营养性吸吮及重力喂养对<32周早产儿全肠道喂养的影响

摘要 目的分析早期非营养性吸吮及重力喂养对胎龄<32周早产儿全肠道喂养的影响。方法选取某院2023年3月至2024年2月收治的200例胎龄<32周的早产儿作为研究对象,采用随机数字表法分为对照组与试验组,每组100例。对照组给予常规喂养,试验组在对照组基础上增加早期非营养性吸吮及重力喂养。观察两组干预前后胃肠道功能、生长发育情况、临床相关指标以及喂养不耐受情况。结果干预后,试验组胃泌素(GAS)以及胃动素(MOT)水平均高于对照组(P<0.05)。试验组体质量、身长以及头围均高于对照组(P<0.05)。试验组胎便排尽时间、胃潴留消失时间、胃食管反流时间以及胃肠道喂养时间均短于对照组,每日排便次数、每日排便量高于对照组,潴留量低于对照组(P<0.05)。试验组喂养不耐受发生率低于对照组(P<0.05)。结论在<32周早产儿全肠道喂养中,早期非营养性吸吮及重力喂养可以更好地促进胃肠道功能恢复,促进早产儿生长发育,且能缩短胃肠道喂养时间,降低喂养不耐受发生率。 Objective To analyze the effects of early non nutritive sucking and gravity feeding on whole gut feeding in premature infants with gestational age<32 weeks.Methods Two hundred premature infants with gestational age<32 weeks admitted to a certain hospital from March 2023 to February 2024 were selected as the study subjects.They were randomly divided into a control group and an experimental group using a random number table method,with 100 cases in each group.The control group received routine feeding,while the experimental group received early non nutritive sucking and gravity feeding in addition to the control group.Observe the gastrointestinal function,growth and development,clinical related indicators,and feeding intolerance before and after intervention in two groups.Results After intervention,the levels of gastrin(GAS)and motilin(MOT)in the experimental group were higher than those in the control group(P<0.05).The body weight,length,and head circumference of the experimental group were higher than those of the control group(P<0.05).The time of fecal exhaustion,the time of disappearance of gastric retention,the time of gastroesophageal reflux,and the time of gastrointestinal feeding in the experimental group were shorter than those in the control group,and the number and amount of daily defecation in the experimental group were higher than those in the control group,while the amount of retention was lower than those in the control group(P<0.05).The incidence of feeding intolerance in the experimental group lower than that in the control group(P<0.05).Conclusions In the whole intestinal feeding of preterm infants<32 weeks,early non-nutritional sucking and gravity feeding can better promote the recovery of gastrointestinal function,promote the growth and development of preterm infants,shorten the gastrointestinal feeding time and reduce the incidence of feeding intolerance.
