妊娠期糖尿病检验中糖化血红蛋白的诊断价值

Diagnostic Value of Glycated Hemoglobin A1c in Gestational Diabetes Mel⁃litus Test

目的探讨妊娠期糖尿病(gestational diabetes mellitus,GDM)孕妇检验中糖化血红蛋白(glycated hemo⁃globin A1c,HbA1c)的诊断价值。方法选取2022年1月—2023年12月漳州市第三医院收治的100例疑似GDM孕妇为研究对象,所有患者均采取HbA1c、空腹血糖(fasting plasma glucose,FPG)检验,以口服葡萄糖耐量试验(oral glucose tolerance test,OGTT)作为金标准,对比不同检验方法的诊断结果和诊断效能。结果疑似GDM患者中,经OGTT诊断出阳性88例,阴性12例;HbA1c检验检出阳性77例,FPG检验检出阳性53例。HbA1c检验与FPG检验的特异度、阳性预测值、阴性预测值比较,差异无统计学意义(P均>0.05)。HbA1c检验的灵敏度、准确度高于FPG检验,差异有统计学意义(P均<0.05)。结论GDM孕妇检验中应用HbA1c检验能够反映血糖控制水平,诊断准确度较FPG检验更高,具有较高的诊断价值。

Objective To explore the diagnostic value of glycated hemoglobin A1c(HbA1c)in pregnant women with gestational diabetes mellitus(GDM).Methods A total of 100 pregnant women with suspected GDM admitted to the Third Hospital of Zhangzhou from January 2022 to December 2023 were selected as the research objects.All patients were tested for HbA1c and fasting plasma glucose(FPG).Oral glucose tolerance test(OGTT)was used as the gold standard to compare the diagnostic results and diagnostic efficacy of different test methods.Results Among the sus⁃pected GDM patients,88 cases were positive and 12 cases were negative diagnosed by OGTT.HbA1c test was positive in 77 cases,and FPG test was positive in 53 cases.There were no significant differences in specificity,positive predic⁃tive value and negative predictive value between HbA1c test and FPG test(all P>0.05).The sensitivity and accuracy of HbA1c test were higher than those of FPG test,and the differences were statistically significant(both P<0.05).Conclusion The application of HbA1c test in GDM pregnant women can reflect the level of blood glucose control,and the diagnostic accuracy is higher than that of FPG test,which has high diagnostic value.
