短期胰岛素强化治疗初发糖尿病的效果探讨

Effect of Short-term Intensive Insulin Therapy on Primary Diabetes Mellitus

目的探讨初发糖尿病经短期胰岛素强化治疗的效果。方法选取2023年1月—2024年4月铜仁市碧江区人民医院收治的86例初发2型糖尿病患者为研究对象,以治疗方法的不同分为常规组和强化组,各43例。常规组采用口服降糖药物治疗,强化组实施短期胰岛素强化治疗,比较两组胰岛功能、血糖指标以及并发症发生情况。结果强化组胰岛素抵抗指数、空腹胰岛素水平、空腹血糖、餐后2 h血糖和糖化血红蛋白均低于常规组,差异有统计学意义(P均<0.05)。强化组胰岛β细胞功能指数水平、餐前1 h和餐后2 h C肽水平高于常规组,差异有统计学意义(P均<0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论短期胰岛素强化治疗初发糖尿病具有更好的疗效,能对患者的血糖进行有效控制,提高患者的胰岛功能。

Objective To explore the effect of short-term intensive insulin therapy on primary diabetes mellitus.Methods A total of 86 patients with primary type 2 diabetes mellitus admitted to Bijiang District People's Hospital of Tongren from January 2023 to April 2024 were selected as the research objects.According to different treatment methods,they were divided into routine group and intensive group,with 43 cases in each group.The routine group was treated with oral hypoglycemic drugs,and the intensive group was treated with short-term insulin intensive therapy.The islet function,blood glucose index and complications were compared between the two groups.Results The homeostatic model assessment of insulin resistance,fasting insulin level,fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the intensive group were lower than those in the routine group,and the differences were statistically significant(all P<0.05).The homeostatic model assessment ofβ-cell function level,1-hour preprandial and 2-hour postprandial C peptide level in the intensive group were higher than those in the routine group,and the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Short-term intensive insulin therapy for primary diabetes mellitus has better curative effect,which can effectively control the blood glucose of patients and improve the islet function of patients.
