基于临床护理路径的干预模式在老年糖尿病合并胆结石患者中的应用研究

Application of Intervention Mode Based on Clinical Nursing Pathway in Elderly Patients with Diabetes Mellitus Complicated with Gallstones

目的探讨基于临床护理路径的干预模式在老年糖尿病合并胆结石患者中的应用效果。方法选取2021年4月—2023年5月安丘市人民医院肝胆外科收治的94例老年糖尿病合并胆结石患者为研究对象,按照不同护理方式分为两组,各47例。对照组采用常规护理方法,观察组联合基于临床护理路径的干预模式,两组均完成4周护理,比较两组血糖水平、自我管理水平及满意度。结果干预后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平较干预前均降低,且观察组低于对照组,差异有统计学意义(P均<0.05)。干预后,两组自我管理水平较干预前均提高,且观察组高于对照组,差异有统计学意义(P均<0.05)。观察组满意度高于对照组,差异有统计学意义(P<0.05)。结论基于临床护理路径的干预模式能有效降低老年糖尿病患者血糖水平,有效缓解由胆结石引起的局部疼痛,患者自我管理水平及满意度较高。

Objective To explore the application effect of intervention mode based on clinical nursing pathway in elderly patients with diabetes mellitus complicated with gallstones.Methods From April 2021 to May 2023,a total of 94 elderly patients with diabetes mellitus complicated with gallstones admitted to the Department of Hepatobiliary Surgery of Anqiu People's Hospital were selected as the research objects.They were divided into two groups according to different nursing methods,47 cases in each group.The control group was treated with routine nursing methods,and the observation group was combined with the intervention mode based on the clinical nursing pathway.Both groups completed 4 weeks of nursing.The blood glucose levels,self-management level and satisfaction were compared between the two groups.Results After intervention,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin A1c in the two groups were lower than those before intervention,and those in the observation group were lower than those in the control group,the differences were statistically significant(all P<0.05).After intervention,the self-management levels of the two groups were higher than those before intervention,and those of the observation group were higher than those of the control group,the differences were statistically significant(all P<0.05).The satisfaction of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The intervention model based on clinical nursing pathway can effectively reduce the blood glucose level of elderly diabetic patients,effectively alleviate the local pain caused by gallstones,and the patient's self-management level and satisfaction are high.
