宁波市镇海区围绝经期综合征中医证型分布研究

Distribution of Traditional Chinese Medicine Syndrome Patterns in Perimenopausal Syndrome in Zhenhai District of Ningbo

目的临床调查统计分析围绝经期综合征(PMS)的中医证候,得出PMS患者的中医证型分布情况,为临床辨证分型和规范化治疗提供依据。方法分析近10年文献中PMS的中医证型分布和常见证候,根据文献和临床实际设计临床PMS中医证候调查表;对399例PMS患者进行临床调查,采用SPSS 26.0进行主成分分析和聚类分析归类证型分布情况;根据聚类分析结果,通过临床专家评审筛选出最优结果,得出最终结论。结果根据聚类分析结果,结合临床实践,认为PMS的中医证型聚类成4类比较合理,即相应的镇海区399例PMS患者证型分布为心肾不交证(32.33%,129/399)、肝肾阴虚证(28.07%,112/399)、肾虚血瘀证(25.31%,101/399)、脾肾两虚证(14.29%,57/399)。结论PMS证型以心肾不交证为主,肝肾阴虚证、肾虚血瘀证次之。

Objective To make clinical investigation and statistical analysis of TCM syndromes of perimenopausal syndrome(PMS)to conclude the distribution of TCM syndrome patterns in perimenopausal syndrome patients,and to provide a basis for clinical differen⁃tiation and standardized treatment.Methods The distribution of TCM syndromes and common symptoms of PMS in the literature in re⁃cent 10 years were analyzed,and the TCM syndromes questionnaire of clinical PMS was designed according to the literature and clini⁃cal practice.A total of 399 patients with PMS were investigated.Principal component analysis and cluster analysis were used to clas⁃sify syndrome distribution by SPSS 26.0.According to the results of cluster analysis,the optimal results were selected through clinical expert review,and the final conclusion was drawn.Results According to the results of cluster analysis and combined with clinical prac⁃tice,it was considered reasonable to cluster the TCM syndrome patterns of PMS into 4 types,that is,the corresponding distribution of 399 cases of PMS patients in Zhenhai District was disharmony between heart and kidney syndrome(32.33%,129/399),liver-kidney yin deficiency syndrome(28.07%,112/399),kidney deficiency and blood stasis syndrome(25.31%,101/39)and deficiency of spleen and kidney syndrome(14.29%,57/399).Conclusion Disharmony between heart and kidney syndrome is the main,liver and kidney yin deficiency syndrome and kidney deficiency and blood stasis syndrome are the second.
