阿帕他胺联合醋酸戈舍瑞林治疗骨转移前列腺癌的有效性和安全性研究

Efficacy and safety study of apatamide in combination with goserelin acetate in the treatment of prostate cancer with bone metastases

目的研究阿帕他胺联合醋酸戈舍瑞林治疗骨转移前列腺癌的有效性和安全性。方法选取2021年5月~2023年5月收治的60例骨转移前列腺癌患者,按照随机数字法分为对照组(n=30)和观察组(n=30),两组患者均采用去势手术治疗,其中对照组术后采用比卡鲁胺治疗,观察组术后采用阿帕他胺联合醋酸戈舍瑞林治疗。两组均坚持治疗3个月。比较两组治疗前后的血清标志物、前列腺症状评分及炎症因子变化,评估临床治疗效果及不良反应。结果治疗后观察组的胰岛素样生长因子-Ⅰ(IGF-Ⅰ)、血管内皮生长因子(VEGF)、前列腺特异性抗原(PSA)水平均低于对照组(P<0.05);治疗后观察组的国际前列腺症状评分(IPSS)均低于对照组(P<0.05);治疗后观察组的白细胞介素-10(IL-10)水平及总有效率高于对照组,肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平低于对照组(P<0.05);两组不良反应对比差异无统计学意义(P>0.05)。结论对于骨转移前列腺癌患者而言,采用阿帕他胺联合醋酸戈舍瑞林治疗可调节患者的血清标志物水平、炎症因子水平,改善前列腺症状,具有较佳的临床疗效,且具有一定安全性。

Objective To study the efficacy and safety of apatamide in combination with goserelin acetate in the treatment of prostate cancer with bone metastases.Methods A total of 60 patients with bone metastatic prostate cancer admitted from May 2021 to May 2023 were selected and divided into the control group(n=30)and the observation group(n=30)according to random number method.Both groups were treated with castration surgery,and the control group was treated with bicalutamide after surgery.The observation group received apaltamide after operation combined with goserrelin acetate.Both groups were treated for 3 months.The changes of serum markers,prostate symptom score and inflammatory factors before and after treatment were compared between the two groups to evaluate the clinical therapeutic effect and adverse reactions.Results After treatment,the levels of IGF-Ⅰ,VEGF and PSA in the observation group were lower than those in the control group(P<0.05).After treatment,IPSS scores in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of IL-10 and total effective rate in the observation group were higher than those in the control group,and the levels of IL-6 and TNF-αwere lower than those in the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion For patients with bone metastatic prostate cancer,apatamide combined with goserelin acetate can regulate the levels of serum markers and inflammatory factors,improve prostate symptoms,and has a good clinical efficacy and certain safety.
