住院患者血流感染病原菌耐药性及危险因素分析

A Study of the Pathogenic Bacterial Resistance and Risk Factors for Bloodstream Infections in Hospitalised Patients

摘要 目的对广州市某三甲医院的住院患者血流感染病原菌耐药性及危险因素进行分析探讨。方法收集2022年4月~2024年3月某院1025例住院患者血培养阳性检出的病原菌及药敏试验资料,采用回顾性流行病学研究方法进行分析。结果血流感染的病原菌主要是大肠埃希菌、肺炎克雷伯菌和金黄色葡萄球菌。耐甲氧西林金黄色葡萄球菌、耐碳青霉烯类大肠埃希菌和耐碳青霉烯类肺炎克雷伯菌感染患者的治疗仍然是临床的一个挑战。结论该院应加强对重症监护室内耐甲氧西林金黄色葡萄球菌、耐碳青霉烯类大肠埃希菌和耐碳青霉烯类肺炎克雷伯菌感染患者的监测和感染防控。 OBJECTIVE The objective of this study is to analyse and discuss the drug resistance of pathogenic bacteria and the risk factors associated with bloodstream infections in patients admitted to a tertiary hospital in Guangzhou.METHODS A retrospective epidemiological study was conducted on data pertaining to 1025 inpatients of a hospital with positively detected pathogenic bacteria in blood culture and drug sensitivity tests.The data were collected from April 2022 to March 2024.RESULTS The most prevalent pathogens associated with bloodstream infections are Escherichia coli,Klebsiella pneumoniae,and Staphylococcus aureus.The continued existence of methicillin-resistant Staphylococcus aureus,carbapenem-resistant Escherichia coli,and carbapenemresistant Klebsiella pneumoniae represents a significant therapeutic challenge.CONCLUSION It is recommended that hospitals implement enhanced surveillance and infection prevention and control measures for patients with methicillin-resistant Staphylococcus aureus,carbapenem-resistant Escherichia coli,and carbapenem-resistant Klebsiella pneumoniae infections in intensive care units.
