摘要
目的:检测晚期世居藏族膝骨关节炎患者血清和关节液中的趋化因子水平差异并探究其临床意义。方法:随机选取2023年5月至2023年8月进行全膝置换术的20例膝骨关节炎患者,采用Luminex多因子检测技术检测膝骨关节炎患者血清以及关节液34项细胞因子的表达水平,采用视觉模拟评分法(VAS)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评估膝骨关节炎患者关节疼痛程度和功能障碍程度,分析膝骨关节炎患者34项细胞因子与VAS评分、WOMAC评分、血红蛋白、C反应蛋白(CRP)、红细胞沉降率(ESR)的相关性。结果:世居藏族膝骨关节炎患者血清和关节液的34项细胞因子中基质细胞衍生因子-1α(SDF-1α)、干扰素诱导蛋白-10(IP-10)、巨噬细胞炎症蛋白-1β(MIP-1β)、单核细胞趋化蛋白-1(MCP-1)的浓度水平比较,差异无统计学意义(P>0.05);关节液组与血清组中SDF-1α、IP-10、Eotaxin、MIP-1β呈正相关(P<0.05);关节液SDF-1α和MCP-1水平与WOMAC评分呈正相关(P<0.05),IP-10与CRP、ESR呈正相关(P<0.05);关节液中的Eotaxin水平与BMI呈负相关(P<0.05);而检测的34个细胞因子水平与VAS评分、WOMAC评分无显著相关(P>0.05)。结论:晚期世居藏族膝骨关节炎患者关节液和外周血中产生了趋化因子,这些趋化因子可能会对早期膝骨关节炎的诊断与治疗提供参考依据。
Objective:To detect the levels of chemokines in serum and joint fluid of late-stage knee osteoarthritis in Tibetan patients and explore their clinical significance.Methods:Twenty patients with knee osteoarthritis who had underwent total knee replacement surgery from May 2023 to August 2023 were randomly selected.Luminex multi factor detection technology was used to detect the expression levels of 34 cytokines in the serum and joint fluid.VAS and WOMAC were used to evaluate the degree of joint pain and functional impairment.The correlation between 34 cytokines and VAS score,WOMAC score,hemoglobin,CRP,and ESR were analyzed.Results:There was no statistically significant difference in the concentration levels of SDF-1α,IP-10,MIP-1β,and MCP-1 among the 34 cytokines in the serum and joint fluid(P>0.05);there was a positive correlation(P<0.05)between SDF-1α,IP-10,Eotaxin,and MIP-1βin the joint fluid group and the serum group;the levels of SDF-1αand MCP-1 in joint fluid were positively correlated with WOMAC score(P<0.05),and IP-10 was positively correlated with CRP and ESR(P<0.05);the level of Eotaxin in joint fluid is negatively correlated with BMI;the levels of 34 cytokines detected were not significantly correlated with VAS score and WOMAC score.Conclusion:Chemokines are produced in the joint fluid and peripheral blood of late-stage knee osteoarthritis in Tibetan patients,which may provide reference for the diagnosis and treatment of early knee osteoarthritis.
