贵阳市疼痛科医师职业安全调查与健康管理研究

摘要 目的了解贵阳市疼痛科医师职业安全与健康(occupational safety and health,OSH)管理情况,为疼痛科医师OSH管理提供依据。方法2021年3月—2023年5月对贵阳市16家医院疼痛科医师年龄、学历、职称、是否作为科室主要负责人进行调查,使用中文版知觉压力量表(Chinese perceived stress scale,CPSS)、Maslach职业倦怠量表(Maslach burnout intentory-general survey,MBI)分析疼痛科医师的压力、倦怠,统计疼痛科医师2020年3月—2021年3月所遭遇的患访暴力次数,并对调查结果进行分析。结果贵阳市疼痛科医师CPSS评分为(40.41±9.96)分。贵阳市疼痛科医师的MBI情绪耗竭、去个性化和职业效能得分分别为(16.45±7.08)分、(16.38±6.40)分和(19.00±8.78)分。职称、是否担任科室负责人与情绪耗竭维度之间呈负相关(r=-0.794、-0.781,P<0.001)。职称与去个性化维度之间呈正相关(r=0.604,P<0.001)、是否担任科室负责人与去个性化维度之间呈负相关(r=-0.662,P<0.001)。职称与职业效能维度之间呈正相关(r=0.623,P<0.001)、是否担任科室负责人与职业效能维度之间呈负相关(r=-0.645,P<0.001)。调查的143名疼痛科医师中2名疼痛医师遭受患访暴力,共收集到3宗患访暴力事故。结论贵阳市疼痛科医师面临一定的职业压力和较高的职业倦怠,住院医师较为明显,同时疼痛科医师同样面临患访暴力。 Objective To investigate the occupational safety and health(OSH)management of pain clinicians in Guiyang City,and to provide a basis for OSH management of pain clinicians.Methods From March 2021 to May 2023,the age,educational background,professional title and whether to be the head of the department of pain clinicians in 16 hospitals in Guiyang City were investigated.The Chinese perceived stress scale(CPSS)and Maslach burnout intentory-general survey(MBI)were used to analyze the stress and burnout of pain clinicians.The number of patient visit violence experienced by pain clinicians from March 2020 to March 2021 was counted and the survey results were analyzed.Results The scores of CPSS score of pain clinicians in Guiyang City was(40.41±9.96)points.The scores of MBI emotional exhaustion,depersonalization and occupational efficacy of pain clinicians in Guiyang City were(16.45±7.08)points,(16.38±6.40)points and(19.00±8.78)points.There was a negative correlation between professional title,whether to be the head of the department and emotional exhaustion dimension(r=-0.794,-0.781,P<0.001).There was a positive correlation between professional title and depersonalization dimension(r=0.604,P<0.001).There was a negative correlation between whether to be the head of the department and depersonalization dimension(r=-0.662,P<0.001).There was a positive correlation between professional title and occupational efficacy dimension(r=0.623,P<0.001).There was a negative correlation between whether to be the head of the department and occupational efficacy dimension(r=-0.645,P<0.001).Among the 143 pain clinicians surveyed,2 pain clinicians suffered violence during visits,and a total of 3 cases of violence during visits were collected.Conclusion The pain clinicians in Guiyang City are facing certain occupational pressure and high job burnout.The residents are more obvious.At the same time,the pain clinicians also face the violence of visiting patients.
