摘要
目的:探究小儿肠系膜淋巴结炎应用高频超声检查的诊断价值。方法:选择2019年1月~2023年1月于本院就诊的60例肠系膜淋巴结炎患儿入研究组,同时纳入60例同期于本院行健康体检的儿童入对照组,上述所有参检儿童均行高频超声检查。分析高频超声检查检出研究组患儿肠系膜淋巴结炎分布情况、血流分级、对比两组不同部位肠系膜淋巴结纵横径之比、最大径。结果:60例患儿经高频超声检查检出脐周部位淋巴结46例,检出率为76.67%(46/60);右下腹部位淋巴结3例,检出率为5.00%(3/60);脐周至右下腹部位淋巴结11例,检出率为18.33%(11/60)。研究组Ⅱ~Ⅲ级血流分级明显高于对照组,0级血流分级低于对照组,有统计学差异(P<0.05)。两组脐周部位、脐周至右下腹部位、右下腹部位肠系膜淋巴结纵横径之比比较,无统计学差异(P>0.05)。研究组脐周至右下腹部位最大径为(23.78±3.62)mm、脐周部位最大径为(19.85±4.19)mm、右下腹部位肠系膜淋巴结最大径为(15.22±3.50)mm,均大于对照组的(5.70±1.46)mm、(7.10±1.34)mm、(4.76±1.75)mm,有统计学差异(P<0.05)。结论:高频超声在小儿肠系膜淋巴结炎诊断中能够有效检出腹部不同部位的淋巴结,通过观察淋巴结的血流情况、纵横径之比、最大径等信息对病情进行有效的判断,可为临床诊疗提供有效的参考。
Objective:To explore the diagnostic value of high-frequency ultrasound in pediatric mesenteric lymphadenitis.Methods:60 children with mesenteric lymphadenitis who visited our hospital from January 2019 to January 2023 were selected into the study group,and 60 children who underwent physical examination during the same period were included in the control group.All the above children underwent high-frequency ultrasound examination.Analyze the distribution and blood flow grading of mesenteric lymphadenitis detected by high-frequency ultrasound examination in the study group of children,compare the ratio of longitudinal and transverse diameters of mesenteric lymph nodes in different parts of the two groups,and determine the maximum diameter.Results:In 60 children,46 cases of peripheral umbilical lymph nodes were detected by high-frequency ultrasound,with a detection rate of 76.67%(46/60);3 cases of right lower abdomen,with detection rate of 5.00%(3/60);11 cases of periumbilical to right lower abdomen,with detection rate of 18.33%(11/60).The study group had significantly higher blood flow grades of II-III compared to the control group,while the 0-grade blood flow grade was lower than the control group,with statistical differences(P<0.05).The ratio of the mesenteric lymph nodes from the umbilical periphery to the right lower abdomen showed no statistical difference(P>0.05).In the study group,the maximum diameter from periumbilical to right lower abdomen was(23.78±3.62)mm,the maximum diameter around umbilical region was(19.85±4.19)mm,and the maximum diameter of mesenteric lymph nodes in right lower abdomen was(15.22±3.50)mm.They were larger than those of the control group[(5.70±1.46)mm,(7.10±1.34)mm,(4.76±1.75)mm],and the differences were statistically significant(P<0.05).Conclusion:High-frequency ultrasound can effectively detect lymph nodes in different parts of the abdomen in the diagnosis of mesenteric lymphadenitis in children.By observing the blood flow of lymph nodes,the ratio of vertical and horizontal diameter and other information of the maximum diameter,it can provide effective reference for clinical diagnosis and treatment.
