摘要
目的:探究脊柱骨折诊断中分别应用1.5T MRI与64排多层螺旋CT的特异度、灵敏度差异。方法:选取2021年9月~2023年9月本院收治的疑似脊柱骨折患者共60例,全部入组对象分别接受1.5T MRI与64排多层螺旋CT检查,并进行手术治疗。金标准以手术结果为准,计算1.5T MRI与64排多层螺旋CT对脊柱骨折的诊断效能。结果:手术结果显示,60例疑似脊柱骨折患者共有21例确诊(阳性),另39例非脊柱骨折患者(阴性)。1.5T MRI阳性与阴性分别检出24例、36例;64排多层螺旋CT分别检出23例、37例。1.5T MRI对脊柱骨折诊断灵敏度、特异度、准确率、阳性预测值、阴性预测值分别是90.48%、87.18%、88.33%、79.17%、94.44%;64排多层螺旋CT对脊柱骨折诊断灵敏度、特异度、准确率、阳性预测值、阴性预测值分别是95.24%、92.31%、93.33%、86.96%、97.30%。两种检查方式上述指标比较,均不存在统计学意义(P<0.05)。结论:脊柱骨折诊断中应用1.5T MRI与64排多层螺旋CT均能够获得理想诊断效果,为疾病诊断与手术治疗提供可靠依据,应结合患者实际情况合理选择。
Objective:To explore the differences in specificity and sensitivity between the use of 1.5T MRI and 64-slice multi-slice spiral CT in the diagnosis of spinal fractures.Methods:A total of 60 patients with suspected spinal fractures admitted to our hospital from September 2021 to September 2023 were selected.All enrolled subjects underwent 1.5T MRI and 64-slice multi-slice spiral CT examinations,and underwent surgical treatment.The gold standard calculates the diagnostic efficacy of 1.5T MRI and 64-slice multi-slice spiral CT for spinal fractures based on surgical results.Results:The surgical results showed that 21 out of 60 suspected spinal fracture patients were confirmed(positive),while the other 39 non spinal fracture patients were negative.24 cases were positive and 36 cases were negative on 1.5T MRI,respectively;64-slice multi-slice spiral CT detected 23 cases and 37 cases respectively.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of 1.5T MRI for the diagnosis of spinal fractures are 90.48%,87.18%,88.33%,79.17%,and 94.44%,respectively;The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of 64-slice multi-slice spiral CT for the diagnosis of spinal fractures are 95.24%,92.31%,93.33%,86.96%,and 97.30%,respectively.There was no statistically significant difference(P<0.05)in the comparison of the above indicators between the two examination methods.Conclusion:The application of 1.5T MRI and 64-slice multi-slice spiral CT in the diagnosis of spinal fractures can achieve ideal diagnostic results,providing reliable basis for disease diagnosis and surgical treatment.Reasonable selection should be made based on the actual situation of patients.
