摘要
目的探讨纤维支气管镜肺泡灌洗联合无创正压通气(NIPPV)对慢阻肺急性加重期(AECOPD)伴呼吸衰竭(RF)患者的治疗效果。方法将80例AECOPD伴RF患者随机分为单一组和联合组各40例。除常规治疗外,单一组行NIPPV治疗,联合组行纤维支气管镜肺泡灌洗联合NIPPV治疗。比较两组的临床疗效、心率、呼吸频率及呼吸困难程度(mMRC评分)。结果联合组治疗总有效率为92.50%,高于单一组的72.50%(P<0.05)。治疗后,两组的心率、呼吸频率及mMRC评分均降低,且联合组心率、呼吸频率及mMRC评分低于单一组(P<0.05)。结论纤维支气管镜肺泡灌洗联合NIPPV治疗AECOPD伴RF患者具有显著效果,可明显改善患者的心率、呼吸频率及呼吸困难程度。
Objective To explore the treatment effect of fiberoptic bronchoscopy alveolar lavage combined with non-invasive positive pressure ventilation(NIPPV)on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with respiratory failure(RF).Methods 80 patients with AECOPD complicated with RF were randomly divided into single group(40 cases)and combined group(40 cases).In addition to routine treatment,the single group was treated with NIPPV,and the combined group was treated with fiberoptic bronchoscopy alveolar lavage combined with NIPPV.The clinical efficacy,heart rate,respiratory rate and dyspnea degree(mMRC score)were compared between the two groups.Results The total effective rate of treatment in the combined group was 92.50%,higher than 72.50%in the single group(P<0.05).After treatment,the heart rate,respiratory rate and mMRC score of the two groups decreased,and the heart rate,respiratory rate and mMRC score of the combined group were lower than those of the single group(P<0.05).Conclusions Fiberoptic bronchoscopy alveolar lavage combined with NIPPV has significant effect in the treatment of patients with AECOPD complicated with RF,and can significantly improve the heart rate,respiratory rate and dyspnea degree.
