摘要
目的分析C反应蛋白/白蛋白(CRP/ALB)比值对输尿管上段结石患者术后尿源性脓毒血症发生的预测价值。方法选取2020年10月至2022年10月行经皮肾镜取石术的输尿管上段结石患者72例,术后2 h检测其血清CRP、ALB水平并计算CRP/ALB比值,统计患者术后1周内尿源性脓毒血症发生情况,比较发生组与未发生组的血清CRP、ALB水平及CRP/ALB比值。ROC曲线分析CRP、ALB及CRP/ALB比值对输尿管上段结石患者术后尿源性脓毒血症发生的预测价值。结果72例输尿管上段结石患者术后尿源性脓毒血症发生7例,占比9.72%。发生组血清CRP水平、CRP/ALB比值高于未发生组,血清ALB水平低于未发生组(P<0.05);绘制ROC曲线结果显示,CRP/ALB比值预测输尿管上段结石患者术后尿源性脓毒血症发生的AUC为0.803,高于CRP、ALB两项指标单独预测。结论CRP/ALB比值对输尿管上段结石患者术后尿源性脓毒血症发生的预测价值较高,值得临床借鉴应用。
Objective To analyze the predictive value of C-reactive protein/albumin(CRP/ALB)ratio for the occurrence of postoperative urinary sepsis in patients with upper ureteral calculi.Methods 72 patients with upper ureteral calculi undergoing percutaneous nephrolithotomy from October 2020 to October 2022 were selected,the serum CRP and ALB levels were detected at 2 hours after operation,and the CRP/ALB ratio was calculated.The incidence of urinary sepsis within 1 week after operation was counted,the serum CRP,ALB and CRP/ALB ratio were compared between the occurrence group and non-occurrence group.ROC curve was drawn to analyze the predictive value of CRP,ALB levels and CRP/ALB ratio for the occurrence of postoperative urinary sepsis in patients with upper ureteral calculi.Results There were 7 cases of urinary sepsis in 72 patients with upper ureteral calculi after operation,accounting for 9.72%.The serum CRP level and CRP/ALB ratio in the occurrence group were higher than those in the non-occurrence group,and the serum ALB level was lower than that in the non-occurrence group(P<0.05).ROC curve showed that the AUC of CRP/ALB ratio in predicting the occurrence of postoperative urinary sepsis in patients with upper ureteral calculi was O.803,higher than the prediction of single detection of CRP and ALB.Conclusions CRP/ALB ratio has high predictive value for the occurrence of postoperative urogenic sepsis in patients with upper ureteral calculi,which is worthy of clinical reference and application.
